Alzheimer&Dementia:血管危险因素、神经影像生物标志物和痴呆发生之间的关系

多种血管和阿尔茨海默氏病理因素的相互作用是神经变性和认知能力下降的基础。

许多研究调查了单个心血管危险因素,包括老年、糖尿病高血压、吸烟和高胆固醇血症对痴呆风险的影响,结果不尽相同。鉴于这些心血管危险因素在个体中聚集并相互影响,如果采用包含这些危险因素的多变量风险预测算法,来探究心血管风险负担对痴呆的整体影响,效果会更好 。

此前使用弗雷明汉一般心血管风险评分(FGCRS)是一种预测算法,通过将人口统计学(即年龄和性别)与传统的心血管风险因素相结合,评估心血管风险负担和患心血管疾病(CVD)的风险。结果显示,FGCRS与认知能力下降有关。

目前,人们对先前的血管危险因素(VRF)状况如何影响晚年的大脑健康知之甚少。为此,来自维克森林医学院内科系的专家分析来自Wake Forest动脉硬化多种族研究核心的不同纵向队列(N = 159;50%非裔美国人,50%白人)的基线VRFs、认知测试和神经影像测量(β-淀粉样蛋白[Aβ]PET,MRI)的关系,结果发表在Alzheimer&Dementia杂志上。

结果显示,与白人相比,非裔美国人表现出基线心血管危险因素、老化和痴呆症发病率(CAIDE)、弗雷明汉中风风险档案(FSRP)和动脉粥样硬化性心血管疾病风险估计(ASCVD)得分更高。其中,观察到在Aβ阳性率、皮质厚度或白质高张力(WMH)体积方面没有明显的种族差异。

较高的基线VRF分数与较低的皮质厚度和较大的WMH体积有关,而FSRP和CAIDE与Aβ有关。Aβ与认知能力有横向联系,所有影像学生物标志物都与更大的6年认知能力下降有关。

该研究结果表明,多种血管和阿尔茨海默氏病理因素的相互作用是神经变性和认知能力下降的基础。
