Radiology:新序列的突破--T1ρ加权动态葡萄糖增强磁共振序列在探究脑实质病变葡萄糖代谢的价值

现如今,脑功能MRI在科研领域是一个争相追逐的热点,不同的脑功能MR方法针对人体不同生理病理性代谢功能进行研究。最近,有研究人员采用7T磁共振扫描仪T1ρ动态葡萄糖增强磁共振成像(DGE-MRI)序列评价其显示颅内血糖浓聚的区域,旨在探究该序列分析肿瘤组织葡萄糖代谢活动的潜能,并将结果发表在影像学权威杂志Radiology上。

现如今,脑功能MRI在科研领域是一个争相追逐的热点,不同的脑功能MR方法针对人体不同生理病理性代谢功能进行研究。最近,有研究人员采用7T磁共振扫描仪T1ρ动态葡萄糖增强磁共振成像(DGE-MRI)序列评价其显示颅内血糖浓聚的区域,旨在探究该序列分析肿瘤组织葡萄糖代谢活动的潜能,并将结果发表在影像学权威杂志Radiology上。

本前瞻性研究自2015年10至2016年7月共纳入了9名新近诊断为胶质瘤的患者以及4名健康志愿者。绝热预化学交换敏感旋转锁定成像序列在7T磁共振扫描仪下进行,能够达到约7秒的时间分辨率。该设备线圈单元覆盖全身。通过肘静脉注射20%的D-葡萄糖100ml并进行T1ρDGE序列扫描。葡萄糖强化是指在DGEρ序列上脑实质信号相对变化区域,并在灰白质间及胶质瘤患者的肿瘤组织和正常白质间进行观察分析。利用Wilcoxon等级秩和检验对所评估脑区域中位信号变化进行分析。

在健康志愿者中,基底神经节灰质中位信号强度(DGEρ= 4.59%)明显高于白质组织(DGEρ= 0.65%)(P = 0.028)。 在病例组,T1ρ加权DGE葡萄糖增强的图像显示肿瘤区域的中位信号强度(DGEρ= 2.02%)显着高于对侧正常白质(DGEρ= 0.08%)(P <0.0001)。

本研究表明,利用T1ρ加权DGE-MRI能够显示健康志愿者和新诊未经治疗的胶质母细胞瘤患者中脑内葡萄糖生理性代谢及病理生理学上葡萄糖摄取活动,从而能够提供肿瘤组织葡萄糖代谢信息的能力。
