卡格列净可降低2型糖尿病和慢性肾脏疾病患者肾功能衰竭的风险,但对特定心血管结局的影响尚不确定,对未患有心血管疾病个体(一级预防)的影响也不确定。 

近日,心血管领域权威杂志Circulation上发表了一篇研究文章,在CREDENCE中,4401名患有2型糖尿病和慢性肾病的参与者在优化的护理基础上被随机分配接受卡格列净或安慰剂治疗。 

与二级预防参与者相比,一级预防参与者(N=2181; 49.6%)更年轻(61 vs. 65岁),女性更常见(37% vs. 31%),并且糖尿病病程较短(15 vs. 16年)( N=2220; 50.4%)。卡格列净总体上降低了主要心血管事件的风险(风险比[HR]为0.80; 95%置信区间[CI]为0.67-0.95; P=0.01),一级预防参与者(HR为0.68; 95%CI为0.49-0.94)和二级预防参与者(HR为0.85; 95%CI为0.69-1.06)都有所降低(相互作用P=0.25)。复合事件成分的影响也相似,包括心血管死亡(HR为0.78; 95%CI为0.61-1.00)、非致死性心肌梗死(HR为0.81; 95%CI为0.59-1.10)和非致死性卒中(HR为0.80; 95%CI为0.56-1.15)。在一级和二级预防中,主要复合肾脏结果和心血管死亡或心力衰竭住院治疗的风险也一直降低(每种结果的相互作用P>0.5)。 

由此可见,卡格列净可显著降低2型糖尿病和慢性肾病患者主要心血管事件以及肾功能衰竭,包括未患心血管疾病的患者。
