ESC 2013:韩雅玲教授发现抗血小板优化治疗可有效降低血小板高反应性

血小板高反应性(HPR)是ACS患者缺血事件的独立预测因子。但是,目前HPR患者的最佳治疗策略并不清楚。为此,沈阳军区韩雅玲教授等开展了这一研究,旨在探索HPR患者安全有效的抗血小板治疗策略。

本项研究共纳入840名血小板高反应性的ACS患者。这些患者被按照1:2的比例随机分配到标准治疗和优化治疗组别。标准治疗组的用药方案为氯吡格雷 75mg/d+阿司匹林300mg/d;优化治疗组患者的抗血小板治疗,先以氯吡格雷150mg/d+阿司匹林300mg/d开始治疗3天,而后研究人员 将按照血小板功能试验的检查结果对患者用药进行调整,PR正常患者继续按照原用药方案治疗,持续HPR患者将被再次随机分配为两组,一组接受西洛他唑50mg/bid+氯吡格雷150mg/d,一组接受西洛他唑100mg/bid+氯吡格雷75mg/d。研究人员在用药30天之后会对所有患者进行血小板功能监测,此后所有患者将继续接受标准用药方案治疗11个月。

研究数据显示,优化治疗组别患者的血小板聚集率以及HPR比例显著低于标准治疗组别;但是对于接受过不同剂量西洛他唑治疗的患者,HPR无显著差异。优化 治疗组别患者2年的死亡率、靶血管血运重建、复发性心绞痛以及主要不良心血管事件均显著低于标准治疗组别。尽管优化治疗组别患者小出血风险与标准治疗组相 比有升高,但总体而言两组患者出血风险相似。

这项研究表明,基于血小板功能监测的抗血小板优化治疗在临床上可行。此外,优化治疗用药方案可有效减弱HPR并可改善ACS患者的长期预后。
