保留幽门的胰十二指肠切除术(PPPD)后,腹部肌肉的切口和缝合会引起炎症变化,引起躯体疼痛,使患者的生活质量下降。近日,研究人员进行了一项临床试验,旨在探究电抽搐肌内刺激(NETOIMS)在腹部开腹手术术后躯体疼痛控制中的效果,研究结果已发表于J Am Coll Surg。

2018年6月至2019年1月,研究人员将44例接受PPPD的患者随机分配至对照组和NETOIMS组。NETOIMS组在全麻下术后即在超声引导下接受腹横肌NETOIMS。从术前1天至出院后2周重复测量疼痛评分(视觉模拟量表)、咳嗽峰值流量(PCF)和步态速度。采用线性混合模型和重复测量方差分析数据。

 

在招募的44名患者中,最终分析了38名患者的数据。分析结果显示,PPPD后NETOIMS组的疼痛评分明显降低(P=0.01)。虽然各测量时间点的PCF未出现组间差异(p = 0.20),但NETOIMS组术后第二天至出院的PCF改善幅度更大(p = 0.02),步态速度改善速度明显快于对照组(p <0.01)。

综上所述,该研究结果表明,NETOIMS有助于快速减轻PPPD术后出现的躯体疼痛,改善PCF和步态速度。
