Brit J Cancer:双膦酸盐与癌症风险

双膦酸盐与降低结直肠癌、乳腺癌和子宫内膜癌的风险显著相关,尤其是含氮双膦酸盐。应当指出,非含氮双膦酸盐可能会增加患肝癌和胰腺癌的风险。

双膦酸盐是否与癌症风险有关尚不清楚。近日,肿瘤领域权威杂志British Journal of Cancer上发表了一篇研究文章,这项荟萃分析旨在评估双膦酸盐对癌症的影响。

研究人员检索了Pubmed、Embase、Cochrane图书馆和Web of Science数据库,检索时间从每个数据库成立到2019年9月26日。研究人员使用随机效应模型计算了含有95%CI的汇总效应估计值,并探索了异质性和发表偏倚。

该研究共纳入了三十四篇文章(4508261名参与者;403196名病例)。分析结果表明双膦酸盐显著降低了结直肠癌(RR=0.89,95%CI:0.81-0.98)、乳腺癌(RR=0.87,95%CI:0.82-0.93)和子宫内膜癌(RR = 0.75,95%CI:0.61-0.94)的风险。但与所有癌症之间未观察到显著相关性。此外,含氮的双膦酸盐仅对乳腺癌(RR=0.94,95%CI:0.90-0.99)和子宫内膜癌(RR=0.70,95%CI:0.54-0.92)有保护作用。非含氮双膦酸盐往往会增加患肝癌(RR=2.14,95%CI:1.23-3.72)和胰腺癌(RR=1.75,95%CI:1.32-2.33)的风险。

由此可见,双膦酸盐与降低结直肠癌、乳腺癌和子宫内膜癌的风险显著相关,尤其是含氮双膦酸盐。应当指出,非含氮双膦酸盐可能会增加患肝癌和胰腺癌的风险。需要进行大规模的前瞻性队列研究来探究双膦酸盐与癌症风险之间的因果关系。
