宽胸气雾剂治疗冠心病心绞痛的网络药理学机制

心绞痛(AP)是冠心病的常见症状,由于冠状动脉循环改变引起暂时性心肌缺血而诱发。

1. 背景

心绞痛(AP)是冠心病的常见症状,由于冠状动脉循环改变引起暂时性心肌缺血而诱发。AP患者发生急性心肌梗死和心力衰竭的风险较高。抗心绞痛药物(如硝酸盐、β受体阻滞剂和钙拮抗剂)是AP治疗的主要药物然而,因其副作用令人担忧。AP患者可能从中医药抗心绞痛治疗中获益。

宽胸气雾剂,由檀香油、高良姜油、荜茇油、细辛油组成。油、细辛油、冰片是抗AP“芳香温热疗法”的传统处方,已被患者广泛接受。

研究表明,宽胸气雾剂具有多种生物活性,包括抗氧化、抗炎、镇静和镇痛特性。但宽胸气雾剂治疗AP的分子机制仍不十分清楚,成为其临床应用的障碍。本研究旨在使用网络药理学方法分析宽胸气雾剂治疗AP的分子机制。

 

2. 方法

网络药理学分析通过质谱裂解模式与NIST17质谱库的比较,在全扫描模式下用GC-MS分析鉴定宽胸气溶胶的成分。

CCK-8法将H9c2细胞(5×103个细胞/孔)接种96孔板培养24h,加入不同浓度的宽胸气雾剂,采用SpectraMaxM5酶标仪在450nm处测定吸光度。

缺氧损伤模型:将H9c2细胞(5×103个细胞/孔)接种于96孔板培养24h。加入宽胸气雾剂,用CCK-8法测定细胞活力。

LPS诱导的炎症模型:将H9c2细胞(5×103个细胞/孔)接种于96孔板培养24h。将细胞预先暴露于宽胸气雾剂1h,然后用LPS(10μg/mL)刺激24h。通过CCK-8法测定细胞活力。将H9c2细胞(5×105个/孔)在6孔板中培养24h,细胞预先暴露于药物1h,然后用LPS(10μg/mL)刺激24h,通过定量实时聚合酶链反应(qPCR)测定H9c2细胞中TNF和IL6的mRNA表达。

H2O2损伤模型:将H9c2细胞(5×103个细胞/孔)接种于96孔板培养24h,细胞预先暴露于硝苯地平和宽胸气雾剂1h,用含H2O的DMEM替换培养基,孵育1h,CCK-8法检测细胞活力。

Pit和ISO诱导的心肌缺血模型:大鼠静脉注射Pit(0.5U/kg)或皮下注射ISO(30mg/kg),分别给予硝酸甘油(0.018mg/kg,i.v.,0.5mL)、普萘洛尔(0.45mg/kg,i.v.,0.5mL)、硝苯地平(0.45mg/kg,i.v.,0.5mL)和宽胸气雾剂(舌下喷雾剂,80μL/kg)。记录大鼠心率(HR)、收缩压(SBP)、舒张压(DBP)。3h后采血,分离血清进行生化分析。实验结束时处死大鼠。摘取心脏组织,用4%多聚甲醛固定,石蜡包埋。切片(厚5μm),HE染色显示心肌细胞形态。

低剂量ISO诱导的心脏纤维化模型:20只雄性Wistar大鼠随机分配至4个实验组。向大鼠(n=5)注射ISO(5mg/kg,s.c.),每日一次,持续7天,以诱导心肌纤维化。大鼠(n=5)给予普萘洛尔(0.45mg/kg,i.v.)或宽胸气雾剂(舌下喷雾剂,80μL/kg)以及ISO 14天。4周后对大鼠进行长轴切面超声心动图检查及左心室内径舒张末期(LVIDd)、收缩末期(LVIDs)、通过VisualSonicsVevo2100成像系统测量舒张末期(EDV)和收缩末期容积(ESV)、每搏输出量(SV)、射血分数(EF)、缩短分数(FS)和HR。实验结束时处死所有大鼠。切取心脏组织切片(厚5μm),HE染色,进行心肌细胞形态观察。Masson三色染色用于评价心脏胶原蛋白纤维。采用ImageJ对图像胶原体积分数(CVF)进行分析。

 

结果

3.1 药理学机制

收集宽胸气雾剂37种主要化合物,从靶标数据库PubChem和预测数据PharmMapper中获得靶标,利用STRING数据库构建靶标与AP相关基因的关联。用Cytoscape软件对蛋白质-蛋白质相互作用网络进行重塑和可视化(图1A)。共提取47个潜在靶标。网络药理学分析表明,宽胸气雾剂的潜在生物标志物和治疗靶点主要包括Fas配体(FASLG)、IL4和过氧化氢酶(CAT),它们与细胞凋亡的调节以及细胞对缺氧、LPS、活性氧(ROS)和机械刺激的反应显著相关(1C)。并且这些蛋白在细胞凋亡、TNF、磷脂酰肌醇-3-激酶-蛋白激酶B(PI3K-Akt)和HIF1信号通路中显著富集(图1D)。

3.2宽胸气雾剂可改善缺氧诱导的H9c2细胞损伤

采用CCK-8法分析宽胸气雾剂的细胞毒性和对缺氧损伤的保护作用。结果表明,0.1-10μL/L浓度的宽胸气雾剂对正常心肌细胞无细胞毒性。与常氧对照组相比,缺氧组细胞活力下降至74%(P<0.05,图2A)。宽胸气雾剂(1和5μL/L)处理可提高低氧条件下12h后的细胞活力(P<0.05,图2B)。通过免疫荧光法进一步测定H9c2细胞中缺氧诱导的HIF1A稳定性,如图2C-D。低氧条件下12hH9c2细胞HIF1A蛋白水平升高,宽胸气雾剂(5μL/L)处理可降低低氧条件下HIF1A水平,但常氧条件下其在宽胸气雾剂处理的H9c2细胞中的水平几乎没有变化。这些结果表明,宽胸气雾剂的保护作用部分是通过其对HIF1A蛋白的作用介导的。

3.3宽胸气雾剂抑制LPS诱导的H9c2细胞炎症

如图2E所示,LPS导致H9c2细胞活力降低约20%,而宽胸气雾剂处理以剂量依赖性方式减弱LPS诱导的细胞毒性。与对照组相比,LPS处理的H9c2细胞中TNF和IL6的mRNA水平显著升高,但在宽胸气雾剂组显著降低(2F,P<0.05)。与正常组相比,LPS处理组可见IL6和TNF的释放增加(P<0.005),而LPS和宽胸气雾剂共同处理组,与LPS组相比,这些蛋白释放减少(2G)。

3.4  宽胸气雾剂可抑制H2O2诱导的H9c2细胞凋亡

利用ABTS和FRAP试验测定油和宽胸气雾剂的抗氧化活性(图3A)。ABTS试验结果发现,这些油的总抗氧化活性为26.7~66.6mM Trolox/mL,FRAP试验结果为79.3~355.1mM Fe2+/mL。宽胸气雾剂能在一定程度上清除氧化自由基,ABTS和FRAP法测定总抗氧化活性分别为20.3 mM Trolox/mL和77.5mmFe2+/mL(图3B)。H9c2细胞暴露于H2O2 (500μM)1h导致细胞活力显著降低(51%)。当用浓度为5μL/L的宽胸气雾剂预处理H9c2细胞时,显示H2O2处理后细胞凋亡显著增加(88%),但宽胸气雾剂处理后细胞凋亡减少(78%,P<0.05),图3C。

3.5 宽胸气雾剂对大鼠心肌缺血的保护作用

给予Pit大鼠硝酸甘油、普萘洛尔、硝苯地平或宽胸气雾剂治疗后,ECG恢复正常,表明其能有效改善AP症状。注射Pit和ISO后大鼠血压和心率立即升高,与其他药物不同,宽胸气雾剂几乎检测不到血压和心率的差异(图4A)。还评价了心肌损伤标志物的活性(图4B)。与正常对照组相比,Pit和ISO处理导致CK、CK-MB和LDH值显著升高。而在Pit-宽胸气雾剂组和ISO-宽胸气雾剂组的CK和LDH的活动显著减少(P<0.05)。

3.6 宽胸气雾剂预防低剂量ISO诱导的心脏纤维化

超声心动图分析显示,与正常组相比,ISO组左心室EF和FS降低(P<0.05),如图6和表1所示。与ISO组相比,普萘洛尔治疗增加了FS(P<0.05)。与ISO组相比,宽胸气雾剂治疗降低了LVIDs(P<0.05),增加了EF和FS,但无显著差异。ISO处理大鼠的心脏组织样本显示大量炎性细胞浸润、心肌紊乱和细胞间隙扩大。宽胸气雾剂治疗14天后,观察到轻微的炎性细胞浸润和完整的心肌细胞膜。病理结果显示ISO组胶原水平增加明显,CVF为18%。与未处理动物相比,宽胸气雾剂处理后胶原水平显著下降(P<0.005)。

结论

本研究基于网络药理学方法分析宽胸气雾剂治疗AP的机制。共确定了47个潜在靶标,其在细胞凋亡调节、细胞对缺氧、LPS、ROS和机械刺激反应方面比较显著,表明宽胸气雾剂的治疗作用是通过其化合物和相关靶标之间的相互作用产生的。实验证据表明,宽胸气雾剂可改善缺氧、炎症和氧化条件下的损伤,进而保护H9c2细胞。宽胸气雾剂对心肌缺血和心肌纤维化表现出保护作用,对血压和心率无明显变化。总之,本研究表明,宽胸气雾剂是一种非血流动力学抗缺血、抗炎、抗氧化剂,具有多组分、多靶点、多途径的特性。
