血管疾病作为全球范围内的主要死因之一,极大地加剧了公共卫生负担。面对心血管疾病发病率持续攀升的挑战,开发高效预防策略显得尤为迫切。高效预测模型作为一种关键手段,运用先进的分析技术和综合评估模型,对个体在未来一段时间内发生心血管疾病(如心脏病发作、中风等)的风险进行准确评估的过程。这一过程涉及多种数据和方法的融合,旨在识别出高风险群体,从而实现早期干预和有效管理,降低心血管事件的实际发生率。

为了开发更高效的心血管事件预测工具,在第33届欧洲高血压学会年会(ESH 2024)上,研究人员通过整合临床数据、生物标志物和已确立的危险因素,开发并评估了一种用于预测心血管事件的模型。尤为重要的是,该模型能预测初次就诊后患者发生心血管事件的可能性。

首先,研究人员通过统计方法、主成分分析法(PCA)和潜在类别分析(LCA)创建变量,从而丰富数据集。随后应用三种不同的预测算法:XGBoost、随机森林和逻辑回归。模型使用70%数据集进行训练,30%用于验证,确保每种算法在预测准确性与可解释性之间取得平衡。该模型旨在预测在基线访问后无心血管疾病史的患者中发生心血管事件的情况。

结果显示,共有4,145名连续患者被转诊至医院高血压科,平均年龄为55.9±14岁,女性占51%。平均随访时间为3,028天,每 100 名患者年的发病密度为 1.9395% CI: 1.78-2.09)。XGBoost 模型的准确率为 85%,灵敏度为 87%,特异性为 71%

当模型缩减到30个最具影响力的变量时,预测准确性略有下降位80%。逻辑回归模型提供了可解释性强的模型,准确率为80%,敏感性为56%,特异性为84%。相比之下,随机森林模型表现出高准确率86%,但灵敏度明显较低,仅为 13%

总之,该研究结果彰显了XGBoost模型的稳健性和稳定性,有效地平衡了预测准确性和可解释性。考虑到模型决策透明度的重要性,具备解释能力的逻辑回归模型的纳入尤为关键。本研究强调了根据应用的具体需求定制预测模型的重要性。未来研究可探索增强XGBoost模型可解释性的可行技术。
