背景:OHCA常见于非可电击节律的患者中(心脏停搏和无脉电活动)。当复苏期间非可电击节律转换为可电击节律时,抗心律失常药物是否是安全和有效尚不明确。

方法:非创伤性OHCA、具备血管通路、≥1次休克后任何时间VF/VT的成年患者,被医护人员前瞻性、随机、双盲分配到胺碘酮、利多卡因或安慰剂组中。已报道了初始表现为电击后顽固性VF/VT的患者。本研究是对独立队列进行的预先定义的分析,该队列初始表现为非可电击OHCA,并随后被随机分配到发展为电击后顽固性VF/VT的患者中。首要结局为出院存活率。次要结局为出院功能状态和药物相关不良反应。

结果:37889例OHCA患者中,3026例初始 VF/VT和1063例初始非可电击转换为可电击节律的患者符合治疗标准,这些患者被随机分组,并接受了他们指定的药物。不同治疗组间非可电击转换为可电击节律患者的基线特征是平衡的,但安慰剂组入组的男性更少,接受心肺复苏的可能性更小。该队列中,阳性药物接受者比安慰剂接受者需要更少的电击、分配药物的额外剂量和辅助抗心律失常药物(P<0.05)。总体来说,16例胺碘酮(4.1%)、11例利多卡因(3.1%)、6例安慰剂(1.9%)治疗的患者存活到出院(P=0.24)。初始OHCA节律(心脏停搏、无脉电活动、VF/VT)中,治疗分组和出院存活之间无显着交互作用。尽管无统计学差异,但阳性药物组生存率均较高,。胺碘酮vs安慰剂的非可电击转换为可电击节律生存率的调整后绝对差异(95% CI)为2.3%(-0.3~4.8;P=0.08),利多卡因vs安慰剂为1.2%(-1.1~3.6;P=0.30)。超过50%的这些存活者功能独立,或需要最少的辅助。药物相关不良反应不常见。
