颅内动脉瘤是颅内动脉壁的局限性病理性扩张,存在破裂倾向。而颅内动脉瘤破裂是自发性蛛网膜下腔出血最常见的病因。蛛网膜下腔出血作为一种对中枢神经系统甚至其他系统造成严重损害的疾病,具有残死率高、临床症状复杂、自然史及转归不明确、治疗方式及效果不佳、并发症多样、随访不统一等一系列问题。自美国心脏协会/美国卒中协会动脉瘤性蛛网膜下腔出血指南发布后,我国先后发布了《中国蛛网膜下腔出血诊治指南2015》《中国蛛网膜下腔出血诊治指南2019》两版指南。破裂颅内动脉瘤（RIA）作为蛛网膜下腔出血的首要病因,其相关高质量研究及先进的诊疗技术陆续发布,而我国关于RIA的规范性诊疗仍缺乏推荐,故专家组结合文献、研究和临床经验共同撰写了本指南,旨在对RIA的病因、诊断、治疗、并发症以及随访等内容进行更新与改写。
