机会性感染 (OIs) 是提醒临床医生注意获得性免疫缺陷综合征 (AIDS) 发生的第一个临床表现。肺孢子菌肺炎 (PCP)、弓形虫脑炎、巨细胞病毒 (CMV) 视网膜炎、隐球菌性脑膜炎、肺结核、播散性鸟分枝杆菌复合体 (MAC) 病和肺炎球菌呼吸道疾病，以及某些癌症，如卡波西肉瘤和中枢神经系统淋巴瘤，有一直是艾滋病的标志。这些 OIs 以及更多的感染平均在感染 HIV 后 7 到 10 年发生。在开发出有效的抗逆转录病毒疗法 (ART) 之前，患者通常在 AIDS 最初表现后仅存活 1 至 2 年。
