  日本研究人员开展的一项最新研究表明,脑钠肽(BNP)水平的降低与坎地沙坦治疗后(必要时加用利尿剂)睡眠时血压的下降相关,无论是否校正清醒时的收缩压下降,而且该相关性不受用药时间影响。研究于2012年3月7日在线发表于《高血压杂志》(J Hypertens.)。

  睡眠时血压已被证实较清醒时血压能更好地反映高血压患者的心血管疾病危险。当前研究旨在评估降压治疗过程中,BNP与睡眠时血压的相关性。

  研究人员评估了J-TOP研究中254例接受动态血压监测的高血压患者,测量患者在基线时和治疗6个月后的BNP水平。J-TOP研究是一项开放性、多中心前瞻性临床研究,该研究比较了家庭测量收缩压值高于135 mmHg的受试者中就寝或清醒时给予坎地沙坦(必要时加用利尿剂)治疗的效果。

  当前研究结果表明:随访期间,对数转换后的BNP测量值的下降与睡眠时收缩压的下降显著相(r =0.27,P <0.001)。即使校正了清醒时的收缩压下降([β]= 0.001,P =0.991),仍存在显著相关性([β]= 0.20,P = 0.002)。将受试者以坎地沙坦治疗时间进行划分后,清醒给药组([β]=0.21,P= 0.028)和就寝给药组([β]= 0.21,p =0.029)的对数转换BNP测量值降低与睡眠时收缩压的下降均显著相关。此外,在接受利尿剂的受试者中此相关性也很强。
