抗原特异性治疗旨在改变1型糖尿病的炎性T细胞反应，恢复免疫耐受。其中，一种策略是将GAD65与GAD-alum结合，利用alum的T helper (Th)2-偏置佐剂特性，从而调节病态的Th1自身免疫。我们在先前报道的由1型糖尿病试验网进行的随机安慰剂对照临床试验中探索了GAD-alum作用的细胞和分子机制。

在由1型糖尿病试验网进行的临床试验中，参与者接种20磅的GAD-alum (2 - 3次)或单独接种GAD-alum，并在基线和治疗后保存外周血单核细胞样本。在本研究中，在这些样品中测量了GAD特异性T细胞反应，并生成了GAD特异性T细胞系和克隆，然后对其进行进一步表征。

免疫后第91天，与单独使用alum进行免疫的参与者相比，我们发现接受GAD-alum免疫的参与者中GAD特异性IL-13 + CD4 T细胞应答的发生率显着更高（分别为71％和94％进行过两次或三次治疗）。 （38％；分别为p = 0.003和p = 0.0002）伴随着高水平的IL-13，IL-4和IL-5分泌，证实了GAD特异性GAD诱导的Th2反应。值得注意的是，免疫后观察到的GAD特异性IL-13 + CD4 T细胞共分泌IFN-γ，表现出双功能Th1 / Th2表型。单细胞转录组分析分别与典型的Th2和Th1转录因子基因GATA3和TBX21一致，确定了IL13和IFNG的表达。

研究结果表明，GAD-alum可免疫激活并具有独特双功能表型的钆特异性CD4 T细胞繁殖，对其进行功能分析可能对理解治疗反应很重要。
