JAHA:速度传递函数是肺动脉硬度新的无创评估手段

肺动脉(PA)硬化与肺血管阻力(PVR)增加有关。PA硬度可通过有创的PA阻抗准确描述。近日,血管疾病领域权威杂志JAHA上发表了一篇研究文章,研究人员假设PA硬度度和阻抗可以通过PA速度传递函数(VTF)来无创地评估,VTF的计算为PA中输出/输入平均速度分布的频谱比率。

在具有临床指证进行右侧心脏导管插入术的20名参与者(55±19岁,14名女性)中,研究人员进行了全面的相位对比和心脏磁共振成像以计算PA的VTF以及右心室的质量和功能。 PA阻抗通过有创的PA压力的频谱比率和超声心动图衍生的PA流量波形来衡量。

参与者的平均PA压力为29.5±13.6 mmHg,PVR为3.5±2.8 Wood单位。混合效应模型显示VTF与PA阻抗显著相关,独立于升高的肺毛细血管楔压(P=0.005)。VTF较高频率模量的平均值与PVR相关(ρ=0.63; P=0.003),并且可以区分PVR较低(n=10)与升高(≥2.5Wood单位,n=10)的对象,曲线下面积为0.95,类似于阻抗的判别能力(曲线下面积=0.93)。VTF与右心室射血分数呈显著负相关(ρ=-0.73; P<0.001),并且与右心室质量指数呈显著正相关(ρ=0.51; P=0.02)。

由此可见,VTF是一种新型的右心室-PA轴偶联参数,是PA阻抗的替代指标,有可能使用心脏磁共振成像无创且可靠地评估PA的硬度和PVR升高。
