大牛再显神威!Carl June发表重磅成果,CAR-T治疗实体瘤或“有戏”

6月21日,Cell旗下Immunity杂志(最新影响因子24.082)发表了题为“Engineered CAR T Cells Targeting the Cancer-Associated Tn-Glycoform of the Membrane Mucin MUC1 Control Adenocarcinoma”的最新研究,Carl H. June及其实验室的博士后研究员Avery D. Posey Jr.是该研究的共同通讯作者。

研究中,科学家们利用基因工程改造人类T细胞,使其能够产生一种能够识别特定糖肽的CAR,这类糖肽在多种癌细胞中表达,但不存在于正常细胞中。研究小组在白血病胰腺癌小鼠模型中证明了这一新型CAR-T疗法的有效性。

随后,通过与哥本哈根大学和芝加哥大学的研究人员合作,科学家小组开发了新型的CAR-T细胞,表达一种叫5E5的单抗,它能够特异性识别一种糖修饰,即mucin 1 (MUC1)蛋白上的Tn多糖,这类修饰不存在于正常细胞,但在癌细胞中非常丰富。简单的说,这是一种靶向癌细胞表面Tn-MUC1(一种新的neoantigen)的新型CAR-T疗法。

研究显示,单抗5E5能够识别多种类型的癌细胞,包括白血病、卵巢癌乳腺癌和胰腺癌细胞,但不会识别正常组织。回输5E5-CAR-T细胞到白血病或胰腺癌小鼠体内降低了肿瘤的生长,提高了生存期。实验结束时,用5E5-CAR-T疗法治疗113后,所有的6只胰腺癌小鼠依然存活。相比之下,到实验结束时,使用非靶向Tn-MUC1的CAR-T疗法治疗的小鼠只有三分之一依然存活。

研究人员表示,这是首次发现一种CAR能够靶向多种不同的实体或液体肿瘤,且对正常细胞没有明显的毒性。至于缺点,Johnson称,这是一种非常新的疗法,有很多与肿瘤水平相关的因素可能会限制治疗。接下来,需要做更多的工作来验证这一疗法在晚期小鼠模型中的安全性,因为这才能更准确的预测在人体中的安全性;并需要证明这一疗法对转移性癌症的疗效,因为这类癌症是癌症相关死亡的主要原因。

如果这些临床前研究均取得成功,科学家们计划进一步开发他们的CAR-T细胞疗法,在即将进行的临床试验中检测它在多种转移性癌症类型中的安全性和有效性。遗憾的是,那位女同事已经去世了。

“小车”甜蜜上路

同期,Immunity杂志还发表了题为“Driving CARs into Sweet Roads: Targeting Glycosylated Antigens in Cancer”的Preview。文章指出,近几年,靶向CD19的CAR-T疗临床试验的成功推动了科学界对肿瘤相关抗原的密集探索,包括癌胚抗原(CEA)、diganglioside GD2、mesothelin以及HER2等。这些抗原有望帮助CAR-T应用于实体瘤,其中还包括了MUC-1,它是粘蛋白家族的一种肿瘤相关糖蛋白,也被用作免疫治疗的靶标。

文章汇总了一些MUC-1相关的CAR-T研究以及Tn相关的先前成果,并详细介绍了这项最新的成果。作者们认为,未来应该进一步探索组合疗法的效力,即结合5E5-CAR-T疗法与免疫检查点阻断剂、特定的趋化因子等。
