T细胞幼淋巴细胞白血病细胞质泡疲累

该患者被诊断为T细胞幼淋巴细胞白血病，并被送入重症监护室。他开始用糖皮质激素治疗，开始了白细胞分离术。

患者为一名64岁的男性，因疲累和头晕6月而到急诊就诊。

体格检查见无明显淋巴结肿大的肝脏和脾脏肿大。实验室检查发现，白细胞计数为每立方毫米1279000（参考范围为4000至11000），淋巴细胞增多为100％，血红蛋白水平为每分升8.6克（参考范围为14至18），血小板计数为24000 立方毫米（参考范围为140000到440000）。外周血涂片发现出许多非典型的中等大小的成熟淋巴样细胞，其具有小的核仁和旋转的细胞质泡（如图A所示）。流式细胞术显示具有CD4和CD8共表达的成熟T细胞群（如图B所示）。通过免疫组织化学分析发现肿瘤淋巴细胞TCL1呈阳性，并且细胞遗传学检查发现14号染色体倒置的复杂核型（q11.2q32），荧光原位杂交显示TCL1重排。



该患者被诊断为T细胞幼淋巴细胞白血病，并被送入重症监护室。他开始用糖皮质激素治疗，开始了白细胞分离术。

随后，他接受了alemtuzumab和pentostatin治疗；3个月后随访时，他的病情完全缓解，残留恶性细胞低于检测阈值（阴性微小残留病）。
