这是一项概念证明型、随机、双盲、安慰剂对照的干预研究，为了评价单次给予40IU的胰岛素和生理盐水对15名糖尿病患者和14名对照参与者血管反应性和认知的影响。测量指标包括区域性灌注、3特斯拉核磁共振成像分析高碳酸血症应激下的血管舒张和神经生理学评估。

结果显示，鼻内胰岛素投药耐受性较好，不会影响全身血糖水平。研究过程中没有严重不良反应事件的报告。鼻内胰岛素改善所有患者的视觉空间记忆(P<0.05)。在糖尿病组中，给予胰岛素后，患者岛叶皮质灌注较对照组增加(P=0.0003)。并且，给予胰岛素后的认知表现与区域性血管反应性有关。糖尿病组患者视觉空间记忆 (R2adj=0.44, P=0.0098)和对照组患者语言流畅性测试的提高(R2adj=0.44, P=0.0087)与大脑中动脉区域血管舒张有关。

研究表明，鼻内胰岛素给药显然是安全的，它不会影响全身血糖控制，对2型糖尿病患者的认知功能起到急性改善作用，其潜在的作用机制可能为血管反应性机制。鼻内胰岛素诱导的认知功能改变可能与脑前区(如调节与注意力有关的任务作业绩效的岛叶皮质)的血管舒张有关。因此，有必要进行大型的研究，以证明鼻内胰岛素疗法的长期作用和鉴别阳性认知反应的预测因子。
