JCEM：妊娠次数与甲状腺自身免疫性疾病的风险有何关系？

自身免疫性甲状腺疾病（AITD）女性比男性更常见。胎儿微嵌合体被认为是这种差异的一个可能的解释近日，内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章，该研究的目的是评估美国人群中女性妊娠次数与甲状腺自身免疫之间的关系。

NHANES调查被用来确定女性抗甲状腺过氧化物酶（TPOAb）、抗甲状腺球蛋白（TGAb）指标以及妊娠次数的数据。受试者（n＝4864）被分为从未妊娠组（n＝909）和既往妊娠组（n＝3955）。研究人员从定性和定量两方面评估了妊娠次数与甲状腺自身抗体之间的关系。甲状腺自身免疫的定义为TPOAb和/或TgAb滴度高于参考范围。

相比于从未妊娠组患者，既往妊娠发生甲状腺自身免疫的比值比[OR]为1.55（95%可信区间为1.26-1.91）。妊娠次数与甲状腺自身免疫有关，妊娠1、2、3和≥4次分别对应的OR为1.37（95%可信区间：1.02-1.84）、1.4（95%可信区间：1.08-1.81）、1.52（95%可信区间：1.18-1.96）和1.73（95%可信区间：1.38-2.18）。既往妊娠女性在年龄、种族、吸烟状况、甲状腺疾病史和尿碘等方面与从未妊娠的女性存在差异（P＜0.001）。多变量回归分析显示妊娠与甲状腺自身免疫无相关性。利用年龄匹配的分析进一步评估这一相关性，证实了甲状腺自身免疫和妊娠次数之间没有存在相关性。
