近期,一项发表在杂志Crit Care Med上的研究旨在评估接受神经肌肉阻滞治疗(患者无法颤抖)的急性呼吸窘迫综合征患者中进行低体温的可行性。

此项研究为回顾性、前瞻性开放标签的可行性研究。

根据柏林标准对58例急性呼吸窘迫综合征患者进行回顾,PaO2 / FIO2小于150的患者接受神经肌肉阻滞治疗。八名PaO2 / FIO2小于150的急性呼吸窘迫综合征患者接受神经肌肉阻滞后进行低温治疗。低温治疗为冷却至34-36℃持续48小时。

此项研究依次测量核心温度、血液动力学、血清葡萄糖和电解质以及P / F,并列出历史对照组和8例冷却患者的中位数(四分位数范围),28天无通气天数和住院死亡率。患者的平均核心温度为36.7℃(36-37.3℃),58例回顾性病例中30例在神经肌肉阻滞治疗期间出现发热。在前瞻性冷却患者中,核心温度达到小于或等于目标范围的时间为4小时,48小时冷却期的92%保持在36℃以下,无不良事件发生,低于对照组(34.35°C [34-34.8℃]; p <0.0001)。与历史对照组相比,冷却患者的医院死亡率降低(75%vs. 53.4%;p = 0.26),无呼吸机天数更长(9 [0-21.5] vs 0 [0-12];p = 0.16) ,3天P / F较高(255 [160-270] vs 171 [120-214];p = 0.024)。

此项研究表明,单独的神经肌肉阻滞不会引起体温过低,而可以使急性呼吸窘迫综合征患者得到有效的冷却。此项结果支持进行急性呼吸窘迫综合征低温随机临床试验,研究急性呼吸窘迫综合征患者接受神经肌肉阻滞的可行性。
