JAMA Inter Med:减肥策略:隔日禁食vs每日热量控制

相比于每日热量限制,隔日禁食参与者依从性差,对于体重控制以及降低心血管事件风险也没有优势

隔日禁食是一种流行的减肥方法但到目前为止还没有长期随机临床试验评估其效果。近日,研究人员比较了隔日禁食与每日热量限制对体重减轻、体重维持和心血管疾病危险因素的影响。

研究参与者为18-64岁,平均BMI 34的肥胖者,研究从2011年10月至2015年1月进行。参与者随机分成隔日禁食组(禁食日摄入25%的能量,非禁食日摄入125%的能量)、热量限制组(每天摄入75%的能量)以及无干预控制组,试验包括6个月的减肥阶段和6个月的体重维持阶段。研究的主要终点是体重变化,次要终点是心血管疾病的饮食干预风险指标以及参与者依从性。

研究招募100名参与者,86名女性14名男性,平均年龄44岁。研究发现,中途放弃率最高的试验组为隔日禁食组,放弃率为38%,热量限制组为29%,对照组为26%。相对于对照组,6及12个月时平均体重减少隔日禁食组与热量限制组相近:6个月时,隔日禁食组vs热量限制组,6.8% vs 6.8%;12个月时,隔日禁食组vs热量限制组,6.0% vs 5.3%;隔日禁食者往往在禁食日吃得比预计的多但在非禁食日却比预计的少,而每日热量限制者通常能达到规定的能量目标。各干预组血压、心率、甘油三酯、空腹血糖、空腹胰岛素、胰岛素抵抗、C-反应蛋白、同型半胱氨酸浓度在6或12个月的差异无统计学意义。相对于每日热量限制组,隔日禁食组的平均高密度脂蛋白胆固醇水平在6个月时显著增加(6.2 mg/dL [95% CI, 0.1-12.4 mg/dL])但12个月时恢复正常, 12个月时禁食组平均低密度脂蛋白胆固醇水平显著升高(11.5 mg/dL [95% CI, 1.9-21.1 mg/dL])。

研究认为,相比于每日热量限制,隔日禁食参与者依从性差,对于体重控制以及降低心血管事件风险也没有优势。
