壁报Abstract #8049:采用靶向二代测序技术鉴定FISH方法未能检测的ALK变异型肺癌。

随着二代测序技术的逐步成熟和成本下降,越来越多的研究和临床实验使用二代测序技术来进行分子分析。来自Foundation Medicine的Siraj Ali报道了临床二代测序分中的ALK阳性肺癌的经验。在1070例肺腺癌的甲醛固定石蜡包埋的样本提取50ngDNA,用于文库构建检测236个癌症相关基因和另外19个基因的的47个内含子序列,平均检测深度达到700X以上。结果发现47例为ALK基因重排的肺癌(4.4%),患者中位年龄52岁,男女比例均等。17例为EML4-ALK V1 融合变体、4例V2,16例V、2例V5、另2例为第18内含子融合。6例为non-EML4-ALK融合变异。在28例 ALK重排患者的标本用FISH分析时,发现9例为FISH阴性(32%),18例为FISH阳性。在9例FISH阴性的患者中,5例有效,2例疾病进展,另2例无法获得疗效资料。结论:NGS技术可能对ALK重排的检测更加敏感。

(2)前瞻性研究显示免疫组化技术可用于常规分析ALK基因表达。

壁报Abstract #8070:来自加拿大 Albert大学的Jean Deschenes报道了Canadian ALK (CALK)结果。

这是一项多中心前瞻性比较同时采用IHC和FISH方法检测 ALK的可靠性。在3个中心采用平行IHC(5A4克隆号的抗体)和FISH 检测了411例临床标本。在373例两种方法均成功分析的标本中,18例(4.8%)在IHC和FISH均为阳性,326例(87.4%)均为阴性。IHC的敏感性和特异性分别为100%和91.8%。提示在前瞻性的研究中IHC可以作为一项可靠方法用于ALK变异检测。

点评:可见对于一次性检测多个基因的方法二代测序技术已经可以覆盖了ALK等重排或融合基因,具有潜在覆盖所有ALK序列变异及重排的优势。在日常工作中,IHC技术可以用于ALK的分析,简便快速,且节约成本。
