目的：Talimogene laherparepvec(T-VEC)是一种单纯疱疹病毒型1-衍生溶瘤细胞的免疫疗法，在肿瘤内选择性地复制，产生巨噬细胞集落刺激因子(GM-CSF)增强系统抗肿瘤免疫反应。一项随机开放的III期临床试验对姑息性阶段IIIB 到IV黑色素瘤患者采用T-VEC与GM-CSF治疗进行了比较。

患者与方法：带有可注射非手术可切除黑色素瘤的患者随机以2:1比率分配，进行病灶内T-VEC或皮下GM-CSF。主要终点是持续的反应率(DRR；客观反应持续不断≥6个月)/每个独立评估。关键的次要终点包括总生存期(OS)和总体反应率。

结果：436名患者随机分配，T-VEC组的DRR(16.3%；95%置信区间，12.1%到20.5%)显著高于GM-CSF组(2.1%；95%置信区间，0%到4.5%；比值比，8.9；P < .001)。T-VEC组的总反应率也更高(26.4%；95%置信区间，21.4%到31.5% v 5.7%；95%置信区间，1.9%到9.5%)。T-VEC组的OS中位数为23.3个月(95% CI，19.5 - 29.6个月)和GM-CSF组的为18.9个月(95%置信区间，16.0至23.7个月)(危险比，0.79；95%置信区间，0.62至1.00；P = .051)。T-VEC对阶段 IIIB，IIIC，或IVM1a 疾病的患者和治疗抵抗疾病的患者作用较为显著。T-VEC最常见的不良反应(AEs)有疲劳、发冷和发热。唯一的3或4级不良反应发生是≥2%的T-VEC-治疗的患者蜂窝织炎(2.1%)。没有发生致命的治疗相关的不良反应。

结论：T-VEC是第一个在III期临床试验中证明溶瘤细胞免疫疗法对黑色素瘤有疗效的治疗方法。T-VEC耐受性良好，而且DRR更高(P < .001)，OS的中位数更长(P = .051)，特别是未经治疗的患者或处于阶段IIIB，IIIC或IVM1a疾病的患者。T-VEC是针对于转移性黑色素瘤患者新型潜在的疗法。
