神经细胞粘附分子1 (NCAM1;CD56)在多达20%的急性髓系白血病(AML)患者中表达。NCAM1广泛用作微小残留病灶的标志物;但NCAM1在AML中的生理功能仍不清楚。

在本研究中,Daniel Sasca等人对NCAM1的表达对白血病发生、耐药性及其作为生物标志物指导治疗的影响进行研究。除了t(8;21)白血病,NCAM1在大多数分子AML亚群中的表达水平具有高度异质性。通过互补基因策略,研究人员发现NCAM1在调节细胞存活和抗应激方面具有重要作用。

无论是在体内还是体外,扰乱NCAM1的表达可诱导细胞死亡或分化,并使白血病细胞对基因毒性药物敏感。此外,在小鼠白血病模型中,Ncam1在白血病祖细胞中高表达,敲除Ncam1可延长疾病的潜伏期,并在连续移植后显著减少白血病起始细胞。

为进一步分析NCAM1相关表型的机制,研究人员对不同的AML细胞系进行蛋白质磷酸化和转录本分析。结果显示NCAM1的表达与调节细胞凋亡或糖基化的MAPK信号通路的固有激活密切相关。用药物抑制MEK1/2可特异性抑制细胞增殖,并使NCA1阳性的AML细胞对化疗敏感。

总而言之,本研究表明NCAM1的异常表达参与白血病干细胞的维持,促进应激耐受,有可能是因为激活了MAPK信号通路。靶向MEK1/2可增加AML细胞对基因毒性药物的敏感性,提示NCAM1可作为指导AML治疗的生物标志物。
