高保真DNA复制是细胞生存的必要条件。复制过程中发生的错误可由DNA聚合酶中的核酸外切酶域和错配修复(MMR)系统修复。复制修复缺陷(RRD)发生在一个或多个由于种系遗传改变而出现缺陷的机制。参与MMR系统的基因杂合子系突变(MSH2、MSH6、MLH1和PMS2等)会导致Lynch综合征。Lynch综合征与结直肠癌的易感性有关,尤其是在成年期,患其他几种癌症的风险也会增加,其中包括脑瘤。导致突变的RRD是高级胶质瘤(HGG)的重要驱动机制,主要发生在RRD相关基因种系突变的背景下。虽然HGG呈现出与癌基因突变相对应的DNA甲基化的特异性模式,但在复制修复缺陷肿瘤中,这一点还没有得到很好的研究。

我们利用450 k或850 k甲基化阵列分析了RRD基因突变背景下出现的51个HGG。这些与HGG进行了比较,并且不知道是否来自RRD患者。RRD·HGG在神经胶质瘤基因如IDH1和H3F3A中有二级突变,显示了与这些甲基化亚组相对应的甲基化模式。值得注意的是,缺乏这些已知次级突变的RRD-HGG与先前标记为"野生C型"或"儿科RTK 1"的一组不完全描述的HGG聚集在一起。对两个对照组HGG队列的独立分析显示,其他RRD/高突变肿瘤聚集在这些亚组中,这表明未诊断的RRD可能是导致HGG聚集在这个位置的原因。与其他癌症中所描述的CpG岛甲基化表型不同,RRD-HGG表现出一种独特的CpG岛去甲基化表型。低甲基化在基因启动子中富集,在对细胞存活至关重要的基因和途径中具有显著的去甲基化作用,包括细胞周期、基因表达、细胞代谢和组织。这些数据表明甲基化阵列可以为RRD HGG的检测提供诊断信息。此外,我们的发现强调了在这些高度失调、过度突变的癌症中独特的自然选择压力,并提供了过度突变和RRD对癌症表观基因组的新影响。

方法:基因集富集分析(GSEA)是利用一个自定义功能对DMPs进行的。排除校正p值大于1×10-5的路径。除了对多重测试进行修正外,还使用missmethy软件包中的"gometh"方法对每个基因中存在的探针数量产生的偏差进行了修正。与生物过程(BP)相关的基因本体论术语被进一步分析,将DMP分析分成每组的高甲基化和低甲基化探针,以确定甲基化差异的方向性。GSEA路径分析使用ReViGO进行整合,计算重叠GO术语的可有性,并将术语列表缩减为分析后保持唯一性的术语。只有分配度<0.5的术语才被纳入图形路径呈现。

总之,我们描述了RRD HGG中一种独特的甲基化模式,这在人类癌症中还没有被描述过。这些发现可能会影响这些患者和家庭成员的诊断、治疗和进一步的基因检测。
