ARD: 超声晕征作为巨细胞动脉炎患者潜在监测工具的前瞻性分析

评估新诊断巨细胞动脉炎(GCA)患者对超声晕圈特征变化的敏感性及其与疾病活动性和糖皮质激素(GC)治疗的相关性。

     目的:颞动脉(TA)和腋动脉(AX)超声显示不可压缩晕征被建议用来确诊疑似巨细胞动脉炎(GCA);然而,它在监测GCA疾病活动方面的价值仍然知之甚少。该研究评估新诊断GCA患者对超声晕圈特征变化的敏感性及其与疾病活动性和糖皮质激素(GC)治疗的相关性

     方法:对在固定时间点接受颞动脉(TA)和腋动脉(AX)连续超声评估的超声确诊GCA 患者进行的前瞻性研究。记录具有晕圈和最大晕圈内中膜厚度(IMT)的节段数。对>80%的患者进行了测量的时间点用于分析。比较了疾病确诊和首次复发时的晕圈特征。

     结果:在354次就诊中对49名患者进行了评估。光晕对变化的敏感性在第1361224周进行评估,显示TA光环特征在所有时间点和基线之间具有显著标准化平均差异,但仅在第6周后才显示AX光环特征具有和基线之间的差异。具有晕圈的TA节段数、总和及最大TA晕圈IMT与下列指标具有显著相关性:红细胞沉降率(0.410.440.48)、C反应蛋白(0.340.390.41)、伯明翰血管炎活动评分(0.290.360.35) GC累积剂量 (-0.34-0.37 -0.32);没有发现AX晕圈特征的显著相关性。晕圈特征出现在94%的首次疾病复发中,但与疾病发作时相比,具有晕圈和晕圈IMT总和的平均节段数较低2.93±1.59 mm vs 4.85±1.51 mmp=0.00122.01±1.13 mm vs 4.49±1.95 mmp=0.0012)。

    结论:超声是一种有用的成像工具,可用于评估巨细胞动脉炎患者的疾病活动度和对治疗的反应。
