临床分期系统已被证明可用于多种神经系统疾病，包括帕金森氏病，多发性硬化症和阿尔茨海默氏病。在肌萎缩性侧索硬化症（ALS）中，大多数临床研究都依赖于总肌萎缩性侧索硬化症的基线变化评分量表-修订（ALSFRS-R）评分为主要终点。尽管功能等级量表（例如ALSFRS-R）在临床试验中可用于评估患者临床状况的变化，但它们可能并未充分强调疾病进展中的重要里程碑。 ALS临床分期系统可能会提供有用的辅助信息。本文是对依达拉奉III期研究MCI186-19（“研究19”）的事后分析，旨在检验临床分期系统作为肌萎缩侧索硬化症（ALS）临床试验结果的效用。
 
方法：肌萎缩性侧索硬化症功能评分量表-研究19的修订项目评分被追溯到金氏阶段和米兰托里诺分期（MiToS）阶段。 作者评估了研究19中King和MiToS阶段进展的患者百分比。作者还根据基线King阶段评估了患者亚组的疾病进展。本研究在日本进行的一项随机、双盲、平行组、安慰剂对照研究；之前已经报告了该研究的详细情况以及对24周双盲治疗期和24周开放标签有效治疗延长期数据的预先指定分析。共有137名患者被随机分为依达拉奉（n=69）或安慰剂（n=68）。
 
结果：在双盲治疗期间，依达拉奉组King阶段进展的患者百分比（42.0%，95%可信区间30.4%至53.6%）低于安慰剂组（55.9%，95%可信区间44.1%至67.6%）。最显著的效果出现在处于第1阶段的患者中，并且在整个治疗过程中保持不变。对MiToS期≥2期进展的分析显示，双盲治疗期间，治疗组与对照组之间无差异，但在开放标记期，安慰剂组与依达拉奉组相比进展更快（对数秩检验，p<0.001）。
 
总之，King和MiToS分期系统为评估依达拉奉研究19的临床进展提供了实用工具。
