HSC引起血液中发现的许多不同细胞类型,可用于治疗各种疾病,包括多发性骨髓瘤,白血病和血液病,如镰状细胞性贫血。 HSC可以从合适的供体收获,然后移植到患者体内,在骨髓自身建立后,可以产生健康的血细胞。

然而,移植过程对于HSC是有压力的,然而,它们中的许多在它们能够成功地固定在患者的骨髓之前死亡。这限制了HSC移植的有效性,延缓了血细胞形成的恢复,增加了感染或出血的风险,甚至导致移植失败。如果供体干细胞的数量较少,HSC死亡是一个特别的问题。例如,脐带血通常含有不足数量的干细胞,用于将其用作移植入成年患者的HSC的来源。

HSC由称为BIM和BMF的两种蛋白质驱动,被称为凋亡过程。永久抑制这两种蛋白质可防止HSC死亡,并提高小鼠HSC移植的效率。但接受这些细胞凋亡抗性干细胞的小鼠很快就会发展出自身免疫性疾病和/或淋巴瘤,因为HSCs及其产生的血液细胞在其应用时不会死亡。

弗莱堡大学医学中心的Miriam Erlacher博士说:“因此,在移植的紧张期间,暂时抑制凋亡可能是改善移植结局而不增加长期不良反应风险的有吸引力的策略。”

Erlacher和同事从小鼠中分离出了HSC,并用遗传工程化的腺病毒感染了它们,这种腺病毒瞬时产生了一种抑制BIM和BMF的称为BCL-XL的人类蛋白质。这些病毒感染的HSC在BCL-XL表达的7-9天对细胞凋亡具有抗性,并且在移植到受体小鼠中时,其在骨髓中建立自身并产生新血细胞的能力大大增强。此外,由于移植的HSC仅表达BCL-XL数日,它们并未促进受体动物中淋巴瘤的形成。

然而,腺病毒感染对HSC有轻微的毒性,因此,Erlacher和同事开发了一种替代方法,其中纯化的BCL-XL可以直接引入到分离的HSC中。第二种方法也提供了临时保护免受细胞凋亡和改善细胞进行移植的能力。

Erlacher说:“我们的研究结果表明,通过操纵供体HSC来暂时抑制细胞凋亡增加了这些细胞的适应性,而不会增加不良病理学的风险。因此,临床凋亡抑制是降低移植物衰竭风险并改善HSC移植结局的有希望的途径。”
