强力霉素不能抑制腹主动脉瘤扩张！JAMA发表美多中心随机临床研究

在腹主动脉瘤的药物治疗方面，强力霉素是研究热点。基础实验显示，强力霉素有助于抑制腹主动脉瘤进展。
 
然而，5月26日发表在JAMA的一项美国随机临床研究提示，强力霉素并不能明显阻止小腹主动脉瘤扩张。
 
作者表示，临床医生应停止用强力霉素作为预防小腹主动脉瘤扩张的手段。
 
该研究覆盖美国22家临床中心，纳入261例≥50岁的小腹主动脉瘤（男士和女士中直径分别为3.5~5 cm和3.5~4.5 cm）患者，随机给予强力霉素胶囊（100 mg口服，每天两次）或安慰剂治疗2年。
 
7例患者没有进行CT随访，最终强力霉素组和安慰剂组分别有129例和125例患者纳入分析，其平均年龄为71岁。
 
2年随访期间，两组中分别有10%和9%的患者接受动脉瘤修复术，死亡率分别为2%和3%。
 
结果显示，两组患者的动脉瘤直径变化没有明显差别。强力霉素组和安慰剂组的基线动脉瘤最大横截面直径均为4.3 cm，2年时动脉瘤直径均增大了0.36 cm。
 
没有患者因为药物不良反应而退出该研究。强力霉素组和安慰剂组分别有65%和63%的患者出现关节疼痛。
