数据收集自2019年11月至2020年10月在我院接受DBS手术的所有患者,除了用于DBS规划的标准3T T1和T2序列外,还可获得7T T2和3T FGATIR MRI。所有患者均同意额外的7T T2 MRI和3T FGATIR序列采集。额外的MRI序列为:(1)7T 3D T2加权和(2)3T 3D FGATIR。术中使用CT进行立体定向定位和电极定位。有关成像参数和手术程序。STN边界由MER确定。在分析中使用了中心MER轨迹。STN的电生理活动被认为是典型的背景信号增宽,伴有强直和不规则的放电模式,偶尔出现突发。腹侧边界的标志是这种典型模式的消失。背侧和腹侧电生理STN边界以mm距离计算。

术后使用Brainlab Elements软件(德国慕尼黑Brainlab AG)对7T T2和3T FGATIR进行STN边界识别,以进行分析。使用轨迹图、轴向和冠状方向的7T T2和3T FGATIR确定MRI序列的边界。相对于目标深度,距离以毫米表示。对于背侧边界识别,轨迹视图图像在0.5秒内提前(有规律地前后移动)。 为了识别腹侧边界,使用7T T2获取STN结束的指示(图1C)。随后,使用3T FGATIR MRI进行明确的边界确定, 直到SN的高信号(与低信号STN相比)变得可见。FGATIR序列仅用于识别带有SN的边界。

显示STN和黑质(SN) T2和3T FGATIR MR和相应的MER。第一排四个面板(垂直方向,A1.1–D1.1)显示了不同深度的直线轨迹视图。第二行(A1.2–D1.2)显示了相应的冠状视图。第三行(A1.3–D1.3)显示了相同的冠状成像,红色轮廓显示STN或SN边界。

共对32名患者的64条轨迹进行了评估。共有17名男性患者,平均手术年龄为62±8岁。除一个微电极外的所有微电极轨迹中,测量了典型的STN活性。这条单一轨迹被排除在边界比较之外。由于2.5%的死亡率,它整个过程都位于MRI STN之外 内侧偏位。在7T T2和3T FGATIR上识别STN背侧边界是可行的。从MRI到MER的平均距离为0.3 毫米(范围-1.0 毫米至+1.0毫米 毫米)和0.2毫米(范围-1.0 毫米至+1.0毫米 mm)分别用于背侧和腹侧边界。MRI上50%的病例的背侧STN边界与MER一致,32%的轨迹位于背侧。61%的患者的腹侧边界与MER吻合,24%的患者的腹侧边界位于背部。STN的平均长度为6.0±1.3 对于MER和6.0±1.2毫米 核磁共振检查。在40%的轨迹中,STN的长度与MRI和MER完全一致。

发现在DBS中,使用7T T2和3T FGATIR MRI可以清楚地定义STN的背侧和腹侧边界。这些边界与MER确定的边界之间的距离平均为0.3 毫米,最大值为1.0 。这些差异很小,被认为与临床无关。STN的电生理描绘和基于成像的电生理描绘的一致性尤为重要。7T T2和3T FGATIR MRI的结合提供了预期STN电生理活动的充分表现。然而,这些序列并不能描述运动节段在STN内的确切位置。在未来,这可能会通过使用7T MRI实现连接性衍生的STN分割来实现。
