Radiology：通过乳腺X线摄影定期复查评估病变进展

本研究旨在比较数字乳腺X线摄影在首轮和再次筛查对乳腺原位癌（DCIS）的检出率及两次的差异，并将结果发表在Radiology上。

本研究共纳入了首次筛查、第一次复诊及第二次复诊中1970例单纯DCIS的患者。筛查时间间隔为22-30个月。利用Logistic回归分析比较首次筛查及再次复诊时不同级别DCIS的检出率。

结果为，所有筛查中，对低级别DCIS的检出率(范围：0.11 [58/508 817例] - 0.25 [178/713 867例] /1000例)最低，而对高级别DCIS的检出率(范围：0.53[271/508 817例] - 0.59 [237/713 867例] /1000例)最高。与首次筛查相比，首次复诊（OR= 0.45， P < .001）和二次复诊（OR = 0.57， P < .001）检出低级别DCIS的概率要低。对中度级别DCIS检出率相对减低更不显着(分别为OR = 0.79，P = .006、OR = 0.76， P = .003)。相反，复查对首次筛查出的高级别DCIS的检出率仍然是在高水平（分别为OR = 0.89， P = .143、OR = 0.97， P = .700）。

本研究表明，复诊对高级别DCIS仍有较高的检出率；相反，对低级别DCIS的检出率较差，而中度DCIS的检出率较首次筛查有所降低。对DCIS检出率与级别相关表明病变的动态变化。
