癌症复发机制揭露,癌细胞可能会进入休眠状态而逃避化学疗法!

治愈癌症还远吗?

急性髓细胞白血病(AML)是所有血癌中最致命的一种,每4个患者中就有3个在5年内死亡。 即使70-80%的AML患者(<60岁)在化疗后实现了临床完全缓解,但大多数患者会通过一种基本隐秘的机制复发。大多数患者在化疗后发生AML复发,预示着疾病无法治愈。

一个主流观点是,AML的复发来自于一个不成熟的耐药亚群,即所谓的白血病干细胞(LSCs)。LSC的概念源于这样的观察:在使用NOD/SCID小鼠的移植实验中,只有具有不成熟免疫表型(CD34+,CD38-)的AML细胞亚群可以重组白血病。然而,最近对小鼠和患者化疗后存活的AML细胞进行分析,在黑点(治疗后化疗持久性癌细胞的剩余部分)并没有发现LSCs的富集,这表明在复发中涉及额外的细微差别。因此,迫切需要更好地了解AML中复发的机制。

由威尔-康奈尔医学的科学家们发表在美国癌症研究协会杂志《癌症发现》上的一项最新研究中显示,癌细胞可以通过进入一种与某种衰老相似的状态来躲避化疗,这是一种 "主动性冬眠",使它们能够抵御旨在破坏癌细胞的侵略性治疗所引起的压力。这种生物过程可以帮助解释为什么癌症在治疗后经常复发。

该研究是在由患者的急性髓细胞白血病(AML)肿瘤样本制成的有机体和小鼠模型中进行的。通过观察在整个治疗和复发过程中收集的急性髓系白血病患者的样本,也验证了这些研究结果。 这些发现对开发可以阻止衰老并使化疗更有效的新药组合具有重要意义。

"急性髓系白血病可以通过化疗进入缓解期,但几乎总是复发,当它复发时,它是无法治愈的,"血液学和医学肿瘤学的Gebroe家族教授,同时也是威尔康奈尔医学的Sandra和Edward Meyer癌症中心的成员Ari M. Melnick博士说"该领域一个长期存在的问题是,'为什么不能把所有的癌细胞都清除掉?除了AML之外,许多其他类型的侵袭性癌症也可以提出类似的问题。"

多年来,癌症研究人员一直在研究肿瘤在看似被化疗完全消灭后如何再次复发。一种理论认为,由于并非肿瘤内的所有细胞在基因水平上都是相同的(一种称为肿瘤异质性的状况),一小部分细胞能够抵抗治疗并重新开始生长。另一种理论涉及肿瘤干细胞的概念,即肿瘤内的某些细胞具有特殊的特性,可使其在化疗后重新形成肿瘤。

在研究中,研究人员发现,当AML细胞暴露于化疗时,有一部分细胞进入了冬眠状态,或者说是衰老状态,而与此同时,该细胞却处于非常类似于炎症的状态。它们看起来类似于经历过损伤的细胞,需要促进伤口愈合--关闭它们的大部分功能,同时募集免疫细胞以使他们恢复健康。

诱导化疗诱导的衰老

"这些特征也常见于发育中的胚胎,由于缺乏营养而暂停了它们的生长,这种状态被称为胚胎停育,"Melnick博士解释说。"这不是一个特殊的过程,而是正常的生物活动,是在肿瘤的背景下发生的。"

进一步的研究显示,这种炎症性衰老状态是由一种叫做ATR的蛋白质诱导的,这表明阻断ATR可能是一种防止癌细胞采用这种状态的一种方法。研究人员在实验室中对这一假设进行了测试,证实在化疗前给白血病细胞服用ATR抑制剂,可以阻止它们进入衰老状态,从而使化疗杀死所有的细胞。

总的来说,研究显示,原发性AML细胞在体外和体内化疗后进入衰老样表型。这伴随着衰老/炎症和胚胎停育转录程序的诱导,MYC和白血病干细胞基因的下调。单细胞RNA-seq提示白血病干细胞在体外和体内的耗竭,并富集了具有明显衰老样细胞的亚群。这种衰老效应是短暂的,并赋予了优越的菌落形成和移植潜力。进入这种衰老样表型依赖于ATR,ATR抑制剂严重损害了AML细胞的持久性。最终结果显示,AML复发是由一种衰老样恢复表型促进的,这种表型的发生与它们的干细胞状态无关。在恢复后,这些衰老后的AML细胞会产生干细胞潜力增加的复发AMLs。

重要的是,另外两个小组同时发表的研究报告指出,衰老的作用不仅对AML很重要,而且对乳腺癌、前列腺癌和胃肠癌的复发病例也很重要,Melnick博士是其中一项研究的合作者。

Melnick博士和他的同事们现在正与生产ATR抑制剂的公司合作,以找到将这些发现转化为临床的方法。然而,还需要进行更多的研究,因为关于何时以及如何给予ATR抑制剂仍存在许多问题。

研究团队表示,对于能够在患者中研究这个问题之前,时机非常关键,因此在实验室中还有很多工作要做。
