Heart：疑似急性心肌梗死患者吸氧有必要吗？

由此可见，虽然吸入氧气较为常见，但是并无重要的临床益处。从八个RCTs的结果支持从血氧正常的患者在临床实践中停止吸入氧气。

虽然氧气疗法被用于疑似急性心肌梗死（AMI）患者的治疗已经一个多世纪，最近的研究已经开始关注血氧正常的患者补充氧气的疗效和安全性。近日，在心脏病领域权威杂志Heart上发表了一篇研究文章，该研究对来自于随机对照试验（RCT）的证据进行合并以探究疑似AIM患者吸入氧气和室内空气的疗效。

研究人员通过检索多个数据库来收集荟萃分析数据，检索时间从成立之时到2017年9月30日。如果研究中在常压下使用任何设备吸入氧气与室内空气进行了比较，无论随访时间长短和结局指标的RCT都被纳入进行分析。研究人员对所有纳入的研究进行评估并提取数据。感兴趣的结局包括死亡率、肌钙蛋白水平、梗死面积、疼痛、低氧血症。

研究人员确定了八个随机对照试验，并进行合并，共纳入了7998例参与者（分别有3982例参与者吸入氧气，4002例参与者吸入空气）。住院和30天死亡的参与者分别为135人，和149人。吸入氧气并没有降低疑似AMI患者住院（比值比为1.11（95%可信区间为0.69-1.77））或30天死亡率（比值比为1.09（95%可信区间为0.80-1.50））的风险，并且在确诊的AMI患者亚组中的结果仍然相似。吸入氧气和未吸入氧气的亚组患者梗死面积（基于心脏核磁共振成像）无显著差异。吸入氧气可降低低氧血症的风险（比值比为0.29（95%可信区间为0.17-0.47））。

由此可见，虽然吸入氧气较为常见，但是并无重要的临床益处。从八个RCTs的结果支持从血氧正常的患者在临床实践中停止吸入氧气。
