研究背景和目的:越来越多的证据表明,肝移植缺血再灌注(I/R)是肝细胞癌复发的危险因素,但是目前机制并不清楚。本研究验证了由于门静脉血流阻断导致的肠系膜阻塞,导致内毒素介导的Toll样受体4(Tlr4)参与,从而导致肝癌负担升高。本研究还评估了远程缺血预处理(RIPC)在这方面的作用。

研究方法:利用肝和股血管闭塞诱导肝I/R损伤和RIPC标准化C57Bl/6j小鼠模型。通过向门静脉注射RIL-175细胞制作肝细胞癌(HCC)模型。在这一背景下,采用增强的Tlr4反应(注射脂多糖)或减弱的Tlr4反应(Tlr4(-/-)小鼠, 肠道杀菌和Tlr4拮抗剂),研究进一步评估了肝肠轴的作用(脂多糖(LPS)- Tlr4信号通路)。

研究结果:门脉夹钳引起上游肠系膜静脉充血和细菌移位,加重了肿瘤负担。RIPC通过保存肠道完整性和减少细菌易位,从而减少HCC复发,从而阻止了这种机制。这些观察结果与LPS-Tlr4通路有关,途径是由具有增强或缺陷的Tlr4反应的小鼠所显示的高、低肿瘤负担所支持的。
