NEJM:分辨多位点基因变异导致的疾病表型

研究者发现在全基因组测序信息丰富,4.9%的病例发现了多种分子诊断。研究结果表明,结构化的临床本体可用于确定同一患者两个不同的遗传疾病的重叠程度;疾病可能被区分开来或是重叠。不同的疾病表型影响不同的器官系统,而重叠的疾病表型很可能是由相互作用在同一通路上的两个基因编码的蛋白质导致的。

全基因组测序为洞察观察到的临床表型和潜在的基因型之间的关系提供了机会。近日,顶级医学期刊NEJM上发表了一篇研究文章,研究人员采用7374例连续不相关的转诊于临床实验室进行全基因组测序的患者数据进行了一项回顾性分析,旨在确定报告有一个以上分子诊断的患者临床特点和频率。

研究人员用人类表型本体中的条款来计算分子诊断与疾病之间的表型相似性。7374例参与者中的2076例(28.2%)进行了分子诊断,在这些患者中,有101例(4.9%)参与者涉及两个或多个疾病位点的诊断。当条件允许的情况下,研究者也分析了父母的样本,发现新的变异占常染色体显性遗传病致病性变异的67.8%(61/90)以及X基因连锁疾病致病性变异的51.7% (15/29);44.7%(17/38)的患者新的变异为两单等位基因变异。12例(11.9%)多个诊断的患者被发现存在因果关系的拷贝数变异。表型来源于两个影响不同的器官系统的不同的遗传疾病患者(50例)的表型相似性得分显著低于有重叠的表型特征疾病的患者(30例)(平均得分,0.21vs.0.36;P=1.77×10-7)。
