【1】Ontuxizumab联合支持疗法治疗耐药转移性结直肠癌

Ontuxizumab(MORAb-004)是一种干扰内皮唾液酸蛋白(肿瘤内皮标志-1)的单克隆抗体。近期,一项随机双盲试验评估了Ontuxizumab治疗耐药转移性结直肠癌患者的安全性和有效性,并根据分子标志鉴别有反应的患者人群。126例患者随机按2:1分配到两组,分别接受每周Ontuxizumab 8mg/kg或安慰剂联合支持疗法直至出现进展或不可耐受的毒性。评估组织和血液标本分子标志识别对Ontuxizumab有反应的患者的能力。结果显示,Ontuxizumab组和安慰剂组在主要研究终点无进展生存方面无显着差异,中位PFS均为8.1周。两组总生存以及总反应率也无显着差异。Ontuxizumab组最常见的治疗相关不良反应为虚弱,恶心,食欲下降和便秘。Ontuxizumab组最常见的3/4级不良反应为背痛。没有分子标志能够识别对Ontuxizumab有反应的患者。

【2】RNF6与结直肠癌不良预后有关

RNF6在肿瘤发生过程中起到重要作用,但是关于结直肠癌中RNF6拷贝数及表达的报道很少。近期一项研究探究了RNF6在结直肠癌发生发展过程中的作用。作者通过公共数据库研究了RNF6的拷贝数和表达情况。结果发现,结直肠癌中RNF6拷贝数显着扩增,该扩增与RNF6表达水平有关。RNF6扩增和过表达与结直肠癌患者不良预后显着相关。基因富集分析表明RNF6高表达的结直肠癌中增殖和侵袭相关基因增加。下调RNF6表达会损伤结直肠癌细胞增殖和侵袭能力。RNF6可能通过诱导SHP-1泛素化和降解激活JAK/STAT3通路,增加pSTAT3水平。

【3】mXELIRI或许可以替换FOLFIRI作为转移性结直肠癌的标准二线疗法

研究表明改良的XELIRI方案用作一线或二线治疗时疗效和耐受性均较好。一项多中心的开放性随机非劣效性的3期临床试验在转移性结直肠癌患者中,将mXELIRI方案与标准FOLFIRI的安全性和疗效进行对比。结果共招募了650位患者,随机分至mXELIRI有或无贝伐单抗组(326人)或FOLFIRI有或无贝伐单抗组(324人)。中位随访15.8个月(IQR 8.7-24.9),共有490人死亡(mXELIRI组242例、FOLFIRI组248例)。 mXELIRI组和FOLFIRI组的总体存活期中位值分别是16.8个月和15.4个月。最常见的3-4级副反应是中性粒细胞减少。mXELIRI组3-4级腹泻的发生率高于FOLFIRI组。mXELIRI组和FOLFIRI组分别报道了46例(15%)和63例(20%)的严重副反应事件。mXELIRI组和FOLFIRI组分别有2例(1例肺炎、1例肺部感染)和1例(肺部感染)治疗相关的死亡。表明mXELIRI结合或不结合贝伐单抗治疗,耐受性良好,相比FOLFIRI结合或不结合贝伐单抗对于总体存活期无劣效性。mXELIRI或许可以替换FOLFIRI作为转移性结直肠癌患者的标准二线主干疗法,至少对亚洲患者是如此。

【4】维生素D转运基因多态性预测转移性结直肠癌患者结局

维生素D可以抑制Wnt信号及血管生成对结肠癌生长产生抑制性影响。那么,涉及到维生素D转运、代谢和信号的基因的SNPs与接受一线FOLFIRI和贝伐单抗治疗的转移性结直肠癌患者临床结局之间是否存在关系尚未可知。研究纳入了522例包括FIRE-3(发现队列)和TRIBE(验证队列)在内的接受FOLFIRI/贝伐单抗治疗的转移性结直肠癌患。278例接受FOLFIRI和西妥昔单抗治疗(FIRE-3)的患者作为对照队列。共分析了6个基因(GC、CYP24A1、CYP27B1、VDR、DKK1、CST5)的SNPs。研究结果表明,在发现队列中,编码维生素结合蛋白的GC rs4588的AA携带者接受FOLFIRI/贝伐单抗治疗后总生存期少于携带有任意C等位基因的患者(15.9 vs 25.1个月),在单因素和多因素分析中均有此差异。这一发现在验证队列的多因素分析中得到验证。有趣的是,对照队列的AA携带者总生存期更长(48.0 vs 25.2个月),这一结果在复发转移性结直肠癌患者接受西妥昔单抗±伊立替康的二次验证队列中得到了验证。

【5】RAS液体活检与转移性结直肠癌患者结局有关

液体活检是转移性结直肠癌患者肿瘤组织RAS检测的替代方案。但是对于接受一线抗EGFR治疗的患者,液体活检RAS检测的预后效果几乎未见报道。CAPRI-GOIM试验对此进行了探究,最终纳入92例患者的肿瘤样本和基线血浆样本NGS数据。在33例患者(35.9%)中肿瘤组织NGS分析和血浆BEAMing检测发现了RAS突变。10例患者为肿瘤组织RAS突变,血浆野生型,还有10例患者为肿瘤组织野生型,血浆突变,最终一致率为78.3%。10例组织突变、血浆野生型的患者中有2例患者的血浆通过ddPCR检测发现RAS突变。在所有的组织野生型、血浆突变患者中,ddPCR或肿瘤组织深度测序均发现等位基因RAS突变的概率在0.15%到1.15%之间。根据组织和液体活检结果确定的RAS突变和野生型患者中位无进展生存时间相似。根据组织和血浆分析确定的RAS突变和野生型患者的中位总生存时间也相似。


【6】纤维摄入与结直肠癌预后的关系

尽管高饮食纤维摄入与结直肠癌风险降低有关,但纤维是否有益于结直肠癌患者生存尚未知。近日,有研究探究了诊断后纤维摄入与死亡率之间的关系。研究共分析了2个研究队列的1575例Ⅰ到Ⅲ期结直肠癌患者。共纳入了1575例患者中,963例为女性,平均年龄68.6岁。中位随访时间8年,共记录773例死亡病例,包括174例死于结直肠癌。确诊后高纤维摄入与死亡率降低有关。患者确诊后纤维摄入高于诊断前水平与死亡率降低有关,摄入增加每5g/d与18%肿瘤特异性死亡和14%总死亡降低有关。从纤维种类看,谷物纤维与结直肠癌特异性死亡和总死亡降低有关,蔬菜纤维与总死亡降低有关,而与结直肠癌特异性死亡无关,水果纤维并未发现与死亡率有联系。全麦摄入与结直肠癌特异性死亡降低有关,校正纤维摄入后,这一有益效果增强。
