Neurology:脑缺血患者溶栓前中性粒细胞高,患者预后差

目的:为了确定缺血性卒中(IS)患者,静脉给予rtPA溶栓前,较高的中性粒细胞计数与症状性脑出血(sICH)和3月不良预后之间的关系。方法:在里尔和赫尔辛基群体研究中,给予IS患者rtPA治疗前,检测血液中白细胞,中性粒细胞以及淋巴细胞计数。主要终点为sICH(欧洲急性脑卒中Ⅱ定义)。次要终点为死亡以及3个月极好预后(mRS分数0-1或等于卒中前),良好预后(mRS分数为0-2或等于卒中前)。结果

结果:共有患者846名(年龄中位数为71岁,50.8%男性)。中性粒细胞计数和中性粒细胞与淋巴细胞比值(NLR)与四种结局独立相关:sICH(校正OR值分别为:1.21,1.11),死亡(校正OR分别为 1.16,1.08),极好预后(校正OR分别为 0.87,0.85)以及良好预后(校正OR分别为 0.86,0.91)。总白细胞计数与四种结局无关。患者sICH最佳判别因子是NLR大于4.80(敏感度66.7%,特异性71.3%,似然比为2.32。与NLR<4.80的患者相比,NLR>4.80的患者发生sICH(95%CI 1.97-6.98)的风险增加3.71倍。

结论:较高的中性粒细胞计数和NLR与患者sICH和3月不良预后独立相关。对这种结果的识别为进行rtPA治疗的患者神经保护提供新的治疗靶向。
