丛状神经纤维瘤是一种良性的周围神经鞘膜肿瘤，是神经纤维瘤的一种组织学亚型。由施万细胞、神经束膜样细胞、成纤维细胞、肥大细胞和残留的有髓或无髓神经以及成纤维细胞组成。

【疾病编码】9550/0

【疾病定义】

  • 是一种良性的周围神经鞘膜肿瘤，是神经纤维瘤的一种组织学亚型。由施万细胞、神经束膜样细胞、成纤维细胞、肥大细胞和残留的有髓或无髓神经以及成纤维细胞组成

【临床特征】

  • 40%的病例伴有1型神经纤维瘤病（NF1）

  • 多发生于青年人

  • 可发生于浅表或深部组织

【大体特征】

  • 结节状和或蠕虫状外观，切面均匀棕褐色/灰褐色，有光泽，黏液样，呈半透明和实性肿瘤

【镜下特征】

  • 结构：呈结节状（蠕虫状），缺之包膜，易浸润下面的软组织和母体神经；

  • 细胞形态：具有细而波浪状核的温和的梭形细胞；

  • 细胞组成：由施万细胞、神经束膜样细胞、成纤维细胞、肥大细胞和残留的有髓或无髓神经以及成纤维细胞组成

  • 背景：为黏液样、胶原性（胡萝卜丝）的间质

【免疫组化】

  • 肿瘤细胞S100，SOX10（+）+间质细胞CD34（+）+残存的轴突NF（+）+散在的神经束膜细胞EMA（+）

【诊断公式】

  • 丛状神经纤维瘤=大体表现（结节状）+波浪状的核+黏液样、胶原性（胡萝卜丝）的间质+免疫组化

【鉴别诊断】

  • 神经鞘瘤

  • 隆突性皮肤纤维肉瘤

  • 神经束膜瘤

【讨论鉴析】

  • 丛状神经纤维瘤是NF1的特征性表现，考试时常会出现牛奶咖啡斑
