BMJ子刊:如何预测2型糖尿病骨骼肌减少风险?这个指标大有用处!

本研究表明Cre/CysC的比值可能是2型糖尿病骨骼肌组成的一个有价值的预测指标。此外,Cre/CysC较高的患者可能有更好的能力处理餐后葡萄糖,大血管疾病的风险较低。

近一项包括1832例2型糖尿病患者和4694例正常血糖受试者的系统综述表明,2型糖尿病患者出现肌少症的风险更高。虑到骨骼肌减少对生活质量的严重影响以及骨折、残疾和死亡风险的增加,它正逐渐成为糖尿病的第三类并发症。

由于肌酸酐与胱抑素C的比值(Cre/CysC)可以反映肌肉体积,它已被证明是糖尿病患者肌肉减少的一个预测指标。本研究通过分析193例2型糖尿病患者腰椎3(L3)水平的CT图像,获得骨骼肌指数(SMI)和平均骨骼肌衰减值(MMA),研究了Cre/CysC对2型糖尿病患者骨骼肌组成的预测价值及其与葡萄糖处理能力和糖尿病并发症的相关性。

结果显示,血清Cre/CysC与SMI和MMA显著相关。多元逐步线性回归分析表明,Cre/CysC是SMI(β=0.48(95%CI 0.02至0.94))和MMA(β=0.57(95%CI 0.14至1.01))的唯一生化预测因子。此外,脂肪质量指数(FMI)与MMA显著相关(r=0.481,p<0.001),而与SMI无关(r=0.101,p=0.164)。

多元逐步线性回归分析与SMI和MMA相关的因素

在糖尿病并发症分析中,患有心血管疾病和下肢动脉疾病的患者Cre/CysC显著降低。此外,在100克馒头试验中,Cre/CysC与餐后60分钟(r=0.162,p=0.045)、120分钟(r=0.287,p<0.001)和180分钟(r=0.313,p<0.001)的血糖水平显著相关。
