维生素A和维生素D是婴幼儿、儿童生长发育所必需的两种重要营养素,与儿童健康息息相关,缺乏或不足将会影响儿童的视觉、骨骼、免疫、造血等系统的发育,甚至导致疾病的发生。随着儿童保健工作的深入开展,我国儿童维生素A、D缺乏的相关研究逐渐成为新的热点。山东省微量元素科学研究会儿童健康专业委员会近期在济南举办了“维生素A、D缺乏防治进展专题研讨会”。会议期间邀请到了多位国内儿童营养、儿童保健和儿科临床领域的知名专家,就儿童维生素A、D缺乏防治新进展及常见问题开展了专题讨论。

视点一   我国儿童维生素A缺乏形势依然严峻

全国14省市6岁以下儿童维生素A缺乏率及边缘缺乏率达50%以上,而6个月以下婴儿缺乏率高达80%。中国居民营养与健康状况调查报告显示,我国3-12岁儿童维生素A缺乏及边缘缺乏率高达50%以上,且农村明显高于城市。2008年针对山东省临沂市的流行病学调查数据显示,7个月-3岁的婴幼儿维生素A缺乏及边缘缺乏率高达60%以上。首都儿科研究所对67例体检儿血清维生素A、25羟维生素D的检测结果显示,低月龄组的婴幼儿的维生素A缺乏率较高(68.8%),纯母乳喂养儿的维生素A和维生素D缺乏率均较高(60.0%、33.3%),这与我国乳母的维生素A营养水平普遍偏低密切相关。

我国维生素A和维生素D营养状况仍不容乐观,应针对高危人群及时施加干预以预防维生素缺乏症的发生。

视点二   我国母乳中维生素A含量远低于世界平均水平

包括中国母乳样本的世界各地产后15天-3月龄母乳样品,维生素A平均水平范围为297-825 ug/L,荟萃分析浓度平均值为444.0±114.6 ug/L。研究显示,中国母乳样本中维生素A的含量远低于世界平均水平,仅为297 ug/L,而实际上国内报告母乳维生素A含量更低,这可能是导致我国婴幼儿普遍维生素A缺乏的原因。

2013年版的《中国居民膳食营养素参考摄入量》婴儿维生素A的AI值是按照世界平均值及我国维生素A理想的目标400 ug/L作为基础计算的,0-6个月婴儿按每日母乳量按照750 ml计算,7-12个月婴儿按每日母乳量按照600 ml计算,而从我国实际的国情来看,纯母乳喂养的婴儿维生素A的摄入量可能更低,远达不到推荐的参考摄入量。

视点三   国际对维生素A补充的建议

目前国际上关于婴幼儿维生素A的补充剂量,采取的是定期大剂量的补充法,即每4-6个月补充一次,每次10-20万IU,针对不同月龄,给予不同的剂量。

补充维生素A的益处在于:在维生素A缺乏地区普遍补充维生素A可使5岁以下儿童死亡率降低23%,孕产妇死亡率降低44%,麻疹患儿补充维生素A后死亡率下降50%。维生素A缺乏的哺乳母亲一次性30万IU维生素A胶囊服用,可提高母亲体内维生素A的贮存量,母乳内维生素A水平和母乳喂养婴儿血清维生素A水平,并使婴儿死亡率降低30%。

视点四   我国对维生素A补充的建议

2013年《维生素A缺乏的诊断、治疗及预防建议》提出了维生素A缺乏的一级预防和二级预防策略:

一级预防:每日膳食中的维生素A摄入量应达到RNI。提倡母乳喂养,并应在出生后及时添加维生素A和D,对母乳不足或无母乳的孩子指导其食用配方奶粉。在维生素A缺乏的高危地区,以及患有麻疹、腹泻、呼吸道感染、水痘等其他严重感染性疾病或蛋白质-能量营养不良的高危人群中执行预防性维生素A补充。

二级预防:针对早期可疑病例,进一步进行相对剂量反应试验(RDR试验)、暗适应检测等,对亚临床及边缘型维生素A缺乏者,除增加膳食中维生素A及β胡萝卜素的摄入外,可每天服用维生素A 1500-2000 IU,与1995年我国《亚临床状态维生素A缺乏的防治方案》中推荐的剂量相同。

视点五  我国对维生素D补充的建议

2015年最新的《维生素D缺乏及维生素D缺乏性佝偻病防治建议》中针对胎儿期、儿童期和高危人群维生素D的补充同样给出了明确的建议:

胎儿期:于妊娠后3个月补充维生素D 800~1000 IU/d,同时服用钙剂。如有条件,应监测血清25-(OH)D浓度,必要时应给予维生素D治疗,使25-(OH)D水平保持在正常范围。

儿童期:户外活动:平均户外活动应在1~2 h/d。6个月以内小婴儿不要直接阳光照射,以免皮肤损伤。维生素D补充:婴儿出生应该尽早开始补充维生素D 400~800 IU/d。

高危人群:早产儿、低出生体重儿、双胎儿生后即应补充维生素D 800~1000 IU/d 。连用3个月后改为D 400~800 IU/d 。

根据健康婴幼儿每日的饮食摄入量进行推算,每日的维生素D摄入量在100 IU左右,与婴幼儿推荐摄入量存在较大差距,因此还需要额外进行补充,避免佝偻病的发生。此外,如果佝偻病的治疗结果不理想,在排除了是否按时服药等因素影响下,还需考虑是否存在其他营养素合并缺乏的情况,如维生素A缺乏、铁缺乏或者锌缺乏。

视点六   维生素AD同补具有协同作用

研究显示维生素A与骨质代谢存在密切关系。维生素A缺乏可使破骨细胞数目减少,成骨细胞的功能失控,导致骨膜骨质过度增生,骨腔变小。维生素D的活性产物1,25-二羟维生素D3能刺激骨细胞与破骨细胞的活动,促进钙的吸收。因此,维生素A与维生素D在促进儿童骨骼健康方面具有协同作用。

维生素A有助于保持呼吸道粘膜完整性,增强上皮组织的防御能力,并可提高免疫细胞功能、促进抗体生成。当维生素A 缺乏可导致机体呼道粘膜上皮细胞的生长和组织修复障碍,防御病原体侵袭的能力降低。维生素D是神经内分泌免疫调节激素,维生素D缺乏可引起机体正常的免疫功能损伤,抗感染能力下降,补充维生素D能够改善患儿免疫功能,提高抗感染能力。因此,维生素A与维生素D在促进儿童免疫功能方面具有协同作用。

由于我国维生素A和维生素D缺乏形势严峻,且维生素A和维生素D在儿童骨骼健康、免疫功能等方面具有协同的作用,因此建议我国维生素A和维生素D的补充采取同补的方式。

视点七   预防剂量的维生素AD是安全的

膳食营养素的参考摄入量(DRI)是一个相对宽泛的范围,在推荐摄入量(RNI)与可耐受最高摄入量(UL)之间都属于有效且绝对安全的摄入范围。很多医务工作者以及广大家长对于维生素A的安全剂量存在疑虑,担心持续每日补充会引起过量或中毒。正是这些误区导致了我国维生素A和维生素D缺乏防治工作存在一定阻力,摄入不足问题在婴幼儿、儿童群体中仍较为普遍,尤其是亚临床缺乏率居高不下。

需要说明的是,6岁以下儿童维生素A的合理摄入范围为1000-3000 IU/d,而中毒剂量为:婴幼儿一次摄入维生素A 30万IU以上即可发生急性中毒;每天摄入5-10万IU,超过6个月即可引起慢性中毒。

目前我国针对维生素A和维生素D缺乏的防治主要是通过服用维生素AD滴剂进行补充(如伊可新)。有些儿保医生或者儿童家长担心AD同补会引起维生素A补充过量。我们通过对市场上10种维生素AD滴剂或者鱼肝油进行了分析,按照维生素A和维生素D 3:1的比例研制的维生素AD滴剂是十分安全的,0-1岁选择服用绿色的胶囊(维生素A含量1500 IU,维生素D 500 IU),1岁以上服用粉色的胶囊(维生素A含量2000 IU,维生素D含量700 IU)。

视点八   维生素A抗感染研究新进展

维生素A又称为“抗感染维生素”,维生素A缺乏与儿童感染性疾病的发生及严重程度密切相关。

维生素A缺乏可引起呼吸道上皮细胞分化及其完整性受损,并降低机体免疫功能,主要机制如下:①当VA缺乏时,呼吸道上皮细胞纤毛消失,出现鳞状上皮化生,腺体细胞功能失常,脱落细胞阻塞管腔,局部防御功能降低,易致病原体侵入,而反复发生感染。②当VA缺乏时,非特异性及特异性免疫系统受到影响,分泌性抗体IgA的生成减少,呼吸道粘膜局部抗感染作用下降,且T、B细胞的活化、增殖、分化功能降低,导致免疫功能受限。③VA缺乏可导致机体免疫功能减弱, 降低儿童的免疫力, 是造成反复呼道感染及哮喘发病的重要原因。同时,呼吸系统感染也可进一步使维生素A水平下降,形成恶性循环。

维生素A缺乏时人体特异及非特异免疫功能受损 , 消化道黏膜完整性遭到破坏, 病原易于侵袭而导致发病, 且发病率和发病程度随维生素A缺乏严重度的增加而加重。补充维生素A可促进肠黏膜上皮细胞的发育,增强对致病微生物的防御能力, 不但可减轻临床症状, 还有助于缩短病程, 减少腹泻复发。

视点九   维生素A在其他领域的研究新进展

维生素A缺乏与贫血的发生也有显著的相关性,缺铁性贫血与血清维生素A水平降低常常同时存在。 可能的机制如下: ①维生素A 缺乏影响运铁蛋白合成,使肝脏储存铁释放入血障碍,引起骨髓缺铁,致造血下降; ②维生素A 缺乏影响亚铁血红素合成时铁的利用,造成幼红细胞增殖分化障碍; ③维生素A 缺乏时机体对感染的易感性增加,从而抑制骨髓造血。

最新研究显示维生素A和维生素D对哮喘发作具有协同调节机制。维生素A对呼吸道上皮细胞的分化及其修复具有重要作用,缺乏维生素A 可使免疫细胞内视黄酸受体的表达下调,从而干扰机体的免疫功能,同时导致呼吸道上皮受损; 反复气道过敏原刺激可致维生素A大量消耗;呼吸道合胞病毒(RSV) 感染还能引起TH1/TH2免疫应答失衡,从而促进哮喘发作。而维生素A可增强TH1的细胞功能,调节并恢复TH1/TH2免疫应答平衡。

维生素A及其活性代谢产物在胎儿肺的分化、增生、发育、成熟及肺损伤的修复中起着至关重要的作用。维生素A是目前治疗支气管发育不良公认的药物( 推荐等级为A)。其主要机制为:①影响肺表面活性物质(PS)合成; ②影响Ⅱ型肺泡上皮细胞的合成; ③影响肺的肺泡化,增加肺泡数量,保持肺泡形态; ④促进肺的血管化。
