Stroke:卒中后运动恢复情况怎么预测?锥体外系支招

纤维束弥散张量成像增加我们对于卒中后不同通路损伤对残余运动功能影响的认识。例如,皮质脊髓束(CST)的完整性有助于了解和预测卒中过程。但目前有关判断基底神经节之间的锥体外系对卒中后运动功能影响的数据较少。Stroke发表的一项最新研究探讨锥体外系完整性与卒中后精细运动功能表现的相关性。

纤维束弥散张量成像增加我们对于卒中后不同通路损伤对残余运动功能影响的认识。例如,皮质脊髓束(CST)的完整性有助于了解和预测卒中过程。但目前有关判断基底神经节之间的锥体外系对卒中后运动功能影响的数据较少。Stroke发表的一项最新研究探讨锥体外系完整性与卒中后精细运动功能表现的相关性。

该研究共计入选26例健康志愿者,行纤维束追踪成像,重建连接黑质与苍白球的黑质-苍白球通路及CST。此外,研究入选20例患者于卒中3个月后评估上述通路及CST并采用各项异性分数评估纤维结构的完整性,于卒中后1年分别采用握力及九孔柱试验分别评估患者的粗细运动能力。此外,为评估影响九孔柱试验的因素,研究还采用多变量模型进行分析。

所有受试者的黑质-苍白球通路均可追踪到,且与CST无重叠,并通过丘脑底核。在卒中患者中,病变同侧的黑质-苍白球通路的各向异性分数显著降低(比率:0.96±0.02,P=0.021);卒中1年后所累及的手行九孔柱试验所需时间明显更长但双手握力相似。进一步分析发现,黑质-苍白球通路的完整性降低、CST完整性降低及年龄较小均与九孔柱试验所示的运动能力较差具有相关性(P分别为0.040、<0.001和<0.001)。

总之,包含连接下丘脑底核的黑质-苍白球通路是锥体外系中非常重要的部分,可在目前已知CST之外影响卒中后的残余精细运动功能,可为预测卒中后运动恢复情况提供支持性信息。
