简要介绍：

慢性心力衰竭（chronic heart failure，CHF）患者常因心功能失代偿需要反复住院治疗，社会和经济负担巨大，已成为严重的全球性健康问题之一。容量负荷过重和肺淤血症状是急性失代偿性心力衰竭（acute decompensated heart failure，ADHF）患者住院的主要原因。液体潴留产生的症状和体征，促使患者住院治疗。充分缓解CHF患者的钠水潴留是减轻症状、降低再住院率、提高生活质量的重要措施，达到干体重也是神经内分泌激素拮抗剂发挥正常疗效的基础。治疗原则是充分清除血管内和血管外组织间隙过剩的体液，同时避免进一步激活神经内分泌系统。现行的CHF处理指南推荐利尿剂为ADHF的一线治疗，利尿治疗能够部分缓解淤血症状，但常不能充分纠正液体潴留，约半数患者出院时仍残存不同程度的淤血表现，这是导致反复因症状复发而住院的主要原因，3个月再住院率高达24%～31%。体外超滤能够根据患者液体潴留程度，可控地清除过剩的体液，是纠正钠水潴留的有效方法。特别是近10年来超滤技术的进步，为临床提供了更好的治疗工具，显示了良好的临床应用前景，已成为CHF利尿剂治疗的重要补充或替代。   

本建议综合了超滤治疗心力衰竭的临床研究进展以及专家的临床治疗经验，以期指导各级医院规范开展这项工作。
