NEJM:乳腺癌相关风险基因评估研究

研究首次定义了与临床上乳腺癌风险相关的基因突变种类,其对乳腺癌遗传风险的评估具有指导意义

乳腺癌高风险基因检测对乳腺癌筛查有益,特别是家族乳腺癌史等高危人群,但许多基因与乳腺癌风险之间的证据较弱,潜在的风险评估不精确,缺乏可靠的亚型特异性风险估计。
 
近日研究人员构架了一个包含34个疑似高风险基因序列集,包含蛋白质截断突变和错义突变,对60466名乳腺癌患者以及53461名对照志愿者进行测序,评估上述风险基因与乳腺癌风险之间的关系
 
研究发现,ATM、BRCA1、BRCA2、CHEK2和PALB2,5个基因中导致蛋白质转录提前终止的截断突变与乳腺癌风险总体相关,P值<0.0001,而BARD1、RAD51C、RAD51D和TP53基因中的截断突变与乳腺癌风险总体相关,P值<0.05,贝叶斯错误发现率<0.05。其余25个基因中19个的蛋白质截断突变,乳腺癌总体优势比的95%置信区间上限<2.0。对于ATM和CHEK2基因中的蛋白质截断突变,其与雌激素受体(ER)阳性乳腺癌的相关性高于ER阴性乳腺癌;对于BARD1、BRCA1、BRCA2、PALB2、RAD51C和RAD51D中的蛋白质截断突变,其与ER阴性乳腺癌的相关性高于ER阳性乳腺癌。总体上,ATM、CHEK2和TP53中的罕见错义突变与乳腺癌风险有关,P值<0.001。BRCA1、BRCA2和TP53基因的错义突变,其风险程度与蛋白质截断突变相似。
