NEJM:感染性脑静脉血栓形成案例报道

女性,71岁,因右耳后出现疼痛,疼痛范围辐射到右眼球后方5天余入院就诊。据该女士主诉,其还伴随有恶臭鼻涕、全身发冷、复视和右眼肿胀等症状。体格检查可见其右上睑下垂、右眼球结膜水肿、视力为20/32、眼球运动功能受限,并伴有三叉神经感觉减退症状(如图A和图B所示)。为了明确该女士的诊断,对其做进一步增强磁共振成像检查,结果显示在其右眼上静脉(图C)、海绵窦(图D)、岩下窦(图E)以及颈内静脉(图F)

据该女士主诉,其还伴随有恶臭鼻涕、全身发冷、复视和右眼肿胀等症状。

体格检查可见其右上睑下垂、右眼球结膜水肿、视力为20/32、眼球运动功能受限,并伴有三叉神经感觉减退症状(如图A和图B所示)。



为了明确该女士的诊断,对其做进一步增强磁共振成像检查,结果显示在其右眼上静脉(图C)、海绵窦(图D)、岩下窦(图E)以及颈内静脉(图F)均出现有部分闭塞性血栓(各图白色箭头所指处)。此外,右侧蝶窦部位发现有脓肿的现象(图红色箭头处)。

血培养和窦道穿刺检验结果显示阴沟肠杆菌阳性。未见该患者有其他明显诱因导致急性鼻窦炎和出现免疫力低下的现象。

最后,主治医生对其进行手术清创和应用抗生素及抗凝治疗,该患者随后恢复良好。

鼻窦感染,一般情况下是通过直接感染或经粘膜静脉板障、大脑系统以及静脉窦等渠道发生感染。但对于本案例中的患者,最有可能是由于静脉窦发生感染导致。
