“酸性体质”会促使癌症恶化与扩散?

“酸性体质”是百病根源?一些网络上的文章强调,癌症是由于吃了过多“酸性食物”导致体质酸化引起的。而所谓“酸性体质”、“硷性体质”的说法常在一些保健食品的宣传内容中出现,不禁让人怀疑其真实性。然而近来科学家的许多研究已经证实,肿瘤内的“乳酸”是造成癌症恶化、扩散的帮凶。

“酸性体质”是百病根源?一些网络上的文章强调,癌症是由于吃了过多“酸性食物”导致体质酸化引起的。而所谓“酸性体质”、“硷性体质”的说法常在一些保健食品的宣传内容中出现,不禁让人怀疑其真实性。然而近来科学家的许多研究已经证实,肿瘤内的“乳酸”是造成癌症恶化、扩散的帮凶。

乳酸是葡萄糖于细胞内新陈代谢以获取能量的过程中所形成的中间产物。在激烈运动期间,肌肉急需大量能量因此快速代谢,造成乳酸堆积在组织和血液中,导致肌肉酸痛与“铁腿”。然而正常的组织与循环系统间有良好的连结,堆积的乳酸可透过血液循环输送至肝脏、肾脏再合成葡萄糖,或是经由代谢消耗掉。

糖在细胞内经过糖解作用(glycolysis)后会产生丙酮酸(pyruvate),“乳酸脱氢酶”则会进一步将丙酮酸转化为乳酸。已知的致癌基因 c-Myc 会促进乳酸脱氢酶基因表现,进而提升癌细胞产生乳酸的能力。然而肿瘤组织内并没有良好的血管连结,乳酸无法顺利经由循环系统输送至肝肾,长期堆积于癌组织的乳酸,会持续作用于癌细胞与周边正常细胞,造成肿瘤恶化。

乳酸促进肿瘤内血管新生

肿瘤体积不断扩大时,许多癌细胞会离供给氧气、养分的微血管越来越远,此时需要生成新的微血管才能满足肿瘤生长所需。先前哈佛大学医学院 Andrius Kazlauskas 教授的研究结果显示,乳酸能促进内皮细胞内血管新生讯息传递,并提升肿瘤内血管新生能力(注 1)。

乳酸抑制 T 细胞功能

研究显示肿瘤组织内乳酸堆积使环境呈酸性,多数细胞无法正常运作,免疫细胞也深受影响。当癌细胞表现高量乳酸脱氢酶时,肿瘤组织内 T 细胞数量会明显下降。体外实验也显示,乳酸会抑制 T 细胞毒杀癌细胞的能力达 50%。值得注意的是,经乳酸处理过的 T 细胞若能培养于正常酸硷环境一段时间,毒杀癌细胞能力又可恢复。

乳酸促进肿瘤内 M2 巨噬细胞形成

M2 巨噬细胞有促进癌症恶化的能力,先前已知癌细胞能诱导 M1 巨噬细胞使之转化为 M2 细胞(注 2)。最近日本国立岐阜大学科学家发现,在头颈癌的肿瘤组织内乳酸含量愈高,则 M2 巨噬细胞的数量也愈高。

人体内的血液是弱硷性的,pH 值约为 7.4,血液 pH 值若太低会出现酸中毒,太高则会出现硷中毒,所以身体过酸或过硷都不好。但所谓“酸性食物”、“硷性食物”对体内酸硷平衡的影响则鲜有科学研究证实,均衡摄取各种食物才是“王道”。至于科学家研究乳酸对癌症恶化的影响,开启了抗癌药物研发新方向,已有相关药物进入临床试验阶段,可望成为人类对抗癌症的新武器。

注 1:血管内皮细胞是构成微血管之最主要组成细胞,当内皮细胞受到刺激时会开始扩散到周围并形成芽状之新生血管,之后并与邻近的血管相连接。

注 2:M1 巨噬细胞具有帮助免疫系统清除肿瘤的功能,但肿瘤恶化时,会透过分泌多种细胞激素,诱导巨噬细胞聚集至肿瘤处,并改造这些巨噬细胞成为癌症的帮凶(M2 巨噬细胞),加速肿瘤的恶化与转移。
