本研究旨在评价心脏MRI T1 和T2 maps纹理分析在诊断急性梗死样心肌炎的价值。

本研究共纳入了39例临床怀疑有急性心肌炎和心梗样表现的患者。受试者行双心室心内膜心肌活检、心脏导管造影和心脏MRI。在心脏MRI中评价T1 和T2 maps和Lake Louise标准。利用免费软件包将纹理分析应用到T1 和T2 maps中。利用逐步降维方法和纹理特征选择选择能够诊断心肌炎的特征,以心内膜心肌活检结果为标准。

结果为,共有26例患者经心内膜心肌活检证实有急性心肌炎,而13例患者无急性炎症特征。平均T1、T2和LLC诊断能力较差,ROC曲线下面积分别为0.65 (95% CI: 0.45, 0.85)、 0.67 (95% CI: 0.49, 0.85)、0.62 (95% CI: 0.42, 0.79)。联合纹理特征宽度不均质和灰度水平不均质能够提高诊断能力,ROC曲线下面积为0.88 (95% CI: 0.73, 1.00) (P < .001),敏感性和特异性分别为89% [95% CI: 81%, 93%] and 92% [95% CI: 77%, 93%]。

本研究表明,T2 maps 纹理分析在诊断急性心梗样心肌炎具有很高敏感性和特异性。
