NEJM:乌拉立肽对急性心力衰竭的效应

对于急性心衰患者,使用乌拉立肽可产生良好的生理效应,但短期治疗并不会影响临床复合终点或降低长期心血管死亡率。

在急性心力衰竭患者中,早期使用静脉血管扩张剂,可以缓解心脏壁压力,减少潜在的心肌损伤,从而有利于患者的预后。研究人员近日就乌拉立肽在急性心力衰竭患者中的应用开展研究。

在一项双盲试验中,研究人员招募了2157名急性心力衰竭患者,随机接受,静脉滴注48小时的,剂量为15ng/kg/min的乌拉立肽或安慰剂。治疗开始后,最初的临床评价中位时间为6小时,主要终点是中位随访15个月的心血管死亡和评价初始48小时临床过程的分层复合终点。

结果显示,心血管死亡在乌拉立肽组发生236例,安慰剂组225例(21.7% vs 21.0%,95% CI 0.85-1.25 P=0.75)。意向治疗分析,没有显着的组间分层复合结果差异。乌拉立肽组较安慰剂组收缩压和N-末端脑钠肽水平下降更多,然而,心肌肌钙蛋白T水平的变化在两组输液过程中没有显着差异。

结果表明,对于急性心衰患者,使用乌拉立肽可产生良好的生理效应,但短期治疗并不会影响临床复合终点或降低长期心血管死亡率。
