JASN:尿铵预测高血压性肾脏疾病的临床疗效

众所周知代谢性酸中毒与CKD预后不良有关。而肾泌铵受损是酸中毒的重要发病机制,所以尿铵排泄可能是比血清碳酸氢盐更好、也许是更早预测酸-基础风险的指标,特别是在无酸中毒的患者中。

近日在JASN上发表的一篇文章则评估了非裔美国人肾脏病和高血压研究中参与者(n=1044)基线排铵与临床结局的关系。平均每日铵排泄为19.5(95%可信区间 [ 95% CI],6.5~43.2)毫克当量。在Cox回归模型(校正人口结构、测定肾小球滤过率、蛋白尿、体重指数、内源性酸产生、血清钾和碳酸氢钠),与较高水平铵排泄相比,死亡或透析复合结局的风险比在较低水平为1.46(95% CI,1.13-1.87)、在中等水平为1.14(95%CI,0.89-1.46)。

基线水平没有酸中毒的参与者中,铵排泄<20毫克当量/天较≥20毫克当量/ 天校正后的危险比为1.36(95% CI,1.09-1.71)。除此之外,与铵排泄高的参与者相比,铵排泄较低的参与者1年酸中毒校正发生率更高(校正后比值比,2.56;95% CI,1.04-6.27)。

由此可见,低铵排泄与高血压肾病患者的死亡和肾衰竭有关,即使在没有酸中毒的患者中也是如此。低铵排泄可识别CKD患者及碳酸氢盐水平正常的患者,这些患者可能在酸中毒发展之前就受益于碱治疗。
