Radiology:增强MRI扫描所用对比剂对人体一点影响也没有吗?

现如今,MRI检查应用的广泛性迎合了临床对常规以及疑难病例诊断的需要,同时强化MRI扫描也越来越普遍。而对于MRI强化扫描时所用对比剂是否对人体有副作用尚未得到证实。经静脉钆对比剂增强检查在一定程度上能够血脑屏障的完整性,研究人员通过观察无病理改变脑实质钆对比剂增强后尸检标本来评估有无对比剂在正常脑实质内沉积,并将这一结果发表在放射权威杂志Radiology上。

现如今,MRI检查应用的广泛性迎合了临床对常规以及疑难病例诊断的需要,同时强化MRI扫描也越来越普遍。而对于MRI强化扫描时所用对比剂是否对人体有副作用尚未得到证实。经静脉钆对比剂增强检查在一定程度上能够血脑屏障的完整性,研究人员通过观察无病理改变脑实质钆对比剂增强后尸检标本来评估有无对比剂在正常脑实质内沉积,并将这一结果发表在放射权威杂志Radiology上。

本研究收集了自2005~2014年共15例患者的尸检脑组织样本,其中将5名接受了4~18次钆增强磁共振(MR)扫描患者的尸检神经元组织样本作为强化对比组,将另10例接受过至少一次MR检查而未进行过对比增强扫描患者的尸检脑组织标本作为对照组。 强化对比组中所用对比剂为钆迪胺。 在受试者的齿状核、脑桥、苍白球和丘脑处取脑组织作为标本,并用电感耦合等离子体质谱(ICP-MS)、透射电子显微镜和能量色散X射线光谱法进行分析,并用光学显微镜定量、定位并评估钆沉积对局部脑组织的影响。

研究结果得出,在增强对比组,4个神经解剖区域脑组织的钆沉积在统计学上具有剂量依赖关系--平均约为0.1-19.4μg/g,(苍白球:ρ=0.90,P =0.04)。相反,ICP-MS在对照组标本中检测不到钆剂沉积。 尸检中所有患者均未发现脑实质病理学改变。 在增强对比组中,有3例患者在行MR检查时肾功能处于临界值(所测肾小球滤过率<45 mL/min/1.73m2)且有肝胆功能障碍。在增强对比组中钆沉积主要局限于毛细血管内皮和神经元间质,并且在两例标本中的上述细胞核内也发现了钆沉积。

本研究表明,无颅内病变的患者经增强对比MR扫描后钆剂会沉积在神经组织中,本研究结果对当前造影剂的生物学分布及安全性提出了挑战。
