心肌酶谱,肌钙蛋白、B型尿钠肽

一、患者准备:

采集空腹静脉血,急诊病人除外。应避免剧烈运动,可以在任何时间段采血。

二、标本采集:

1、采集者需遵照医嘱准备采集容器,打印条码并正确粘贴条码于容器上。

2、采集容器:真空采血管红色或黄色(

3、采集方法:静脉采血参考护理有关操作规程。避免用力震荡试管造成标本溶血。不能在输液侧采血避免血液被稀释导致测值偏低或出现假阴性。

4、采集量:静脉血3ml。

5、采集后保存及运输:标本应无溶血、无脂血,采集后立即室温运送至检验科。

三、注意事项:

1、运动后肌红蛋白升高。

2、污染、溶血、乳糜血标本会干扰实验结果。

3、溶血对检测结果影响较大,使测定结果远高于真值。

四、医生解读:

1、心肌酶谱:

肌酸激酶的参考范围与临床意义:

【参考范围】(方法:速率法)

男: 38~174U/L; 女: 26~140U/L

【临床意义】

肌酸激酶(CK)水平受性别、年龄、种族、生理状态的影响。①男性肌肉容量大,CK活性高于女性;②新生儿出生时由于骨骼肌损伤和暂时性缺氧,可使CK升高;③黑人CK约为白人的1.5倍;④运动后可导致CK明显增高,且运动越剧烈、时间越长,则CK升高越明显。

CK增高:①急性心梗时CK水平在发病3~8小时即明显升高,其峰值在10~36小时,3~4天恢复正常。如果在病程中CK再次升高,提示心肌再次梗死。因此CK为早期诊断急性心梗的灵敏指标之一,但诊断时应注意CK的时效性。发病8小时内CK不增高,不可轻易排除心梗,应继续动态观察;发病24小时的CK诊断价值最大,此时CK应达峰值,如果CK小于参考值上限,可排除心梗。但应除外CK基础值极低的病人和心肌梗死范围小及心内膜下心肌梗死等,此时即使心肌梗死,CK也可正常。②心肌炎和各种肌肉疾病时CK可明显升高;③溶栓治疗:急性心梗溶栓治疗后出现再灌注,导致CK活性升高,有助于判断溶栓后再灌注的情况;④心脏手术和非心脏手术都可导致CK增高,其增高程度与肌肉损伤程度,手术时间、手术范围有密切关系。

CK减低:长期卧床、甲状腺功能亢进症、激素治疗等CK均减低。

肌酸激酶同功酶MB的参考范围与临床意义:

【参考范围】(方法:速率法)0~20U/L

【临床意义】

肌酸激酶同工酶MB为心肌相对特异性酶,主要用于急性心梗诊断, CK和CK-MB一起检测有助于心肌梗死的诊断和监视。酶质量法测定优于酶活性测定。

CK-MB对急性心梗早期诊断的灵敏度明显高于总CK,其阳性检出率达100%,且具有高度的特异性。其灵敏度为17~62%,特异性为92~100%。CK-MB一般在发病后3~8小时增高,9~30小时达高峰,48~72小时恢复正常。与CK比较,其高峰出现早,消失较快,对诊断发病较长时间的心梗有困难,但对心肌再梗死的诊断有重要价值。另外,CK-MB高峰时间与预后有一定关系,CK-MB高峰出现早者较出现晚者预后好。

其他心肌损伤,如心绞痛、心包炎、慢性心房颤动、安装起搏器等,CK-MB也可增高。肌肉疾病及手术,骨骼肌疾病时,CK-MB也增高,但CK-MB/CK常小于6%,以此可与心肌损伤鉴别。

血谷草转氨酶测定的参考范围与临床意义:

【参考范围】(方法:速率法)8.0~40.0U/L

【临床意义】

天门冬氨酸氨基转移酶(AST)旧称谷草转氨酶(GOT)。AST水平的升高和心肌或骨骼肌损伤,以及肝组织损害等都有关,而ALT是肝脏的特异性酶,仅在肝胆疾病时显著升高。因此同时进行ALT和AST的检测用于鉴别肝损伤和心肌或骨骼肌损伤。AST/ALT比率可用于肝病的鉴别诊断。比率<1预示中度的肝损伤;比率>1和严重肝病有关,常见慢性肝病。

血清乳酸脱氢酶测定的参考范围与临床意义:

有五种同功酶LDH1-LDH5。急性心梗12-24hLDH升高,48-72h达高峰,1-2周后恢复正常。

LDH同功酶 LDH有两个亚单体:H(心),M(肌肉),组成的四聚体。

a.以LDH1、LDH2为主:心,肾,脑,睾丸,红细胞

b.以LDH5、LDH4为主:肝,骨骼肌,肠粘膜

c.以LDH3为主:肺,脾,淋巴结,内分泌腺,血小板,非妊娠子宫

恶性肿瘤晚期患者胸腹水中血清乳酸脱氢酶升高。慢性肾小球肾炎、SLE、糖尿病肾病等病人中,尿血清乳酸脱氢酶可达正常人3-6倍,尿中含多种抑制血清乳酸脱氢酶活性物质,如尿素、小分子肽类、低PH值也可抑制LDH活性。尿毒症患者LDH正常,透析后血清乳酸脱氢酶升高。

血清α-羟丁酸脱氢酶(a-HBD)测定的参考范围与临床意义:

血清α羟丁酸脱氢酶(a-HBDH)不是一种独立的特异酶,而是含有H 亚基的LD-1和LD-2的总称。由肝细胞线粒体合成,其在体内分布较广,其体内的含量依次为肾、胰、肝、脾。

急性心梗a-HBDH活力升高;

正常人a-HBDH/LDH=0.67;

急性心梗a-HBDH/LDH≥0.8;

肝脏疾病a-HBDH/LDH<0.6。

2、肌钙蛋白-I定量(cTn-I)

肌钙蛋白分子量较小,心肌损伤后游离的肌钙蛋白从心肌细胞释放入血,使血清中的浓度迅速升高,其升高的倍数往往超过CK 或CK-MB的变化。其升高的时间与CK-MB相似,但持续时间较长。肌钙蛋白有肌钙蛋白T和肌钙蛋白I两种亚型,都是急性心肌梗死诊断的确定标志物。

只要血清中肌钙蛋白I超过0.04ng/ml便认为心肌有损伤,>0.1ng/ml必须对心肌损伤采取措施,是目前诊断心梗效率最高的检测指标之一。尤其是对ST段非抬高的不稳定性心绞痛危险分层和预后判断有重要意义。

急性心梗的诊断:肌钙蛋白I在急性心梗发病后3~6小时即升高,14~20小时达到峰值,5~7天恢复正常。其诊断急性心梗的灵敏度为6~44%,特异性为93~99%。

判断微小心肌损伤:不稳定性心绞痛患者常发生微小心肌损伤,血清肌钙蛋白I 水平升高,提示心肌有小范围的梗死。

其他:急性心肌炎病人肌钙蛋白I水平升高,其阳性率达88%,但多为低水平增高。

3、B型尿钠肽(BNP)的检测

BNP: <300 pg/ml  

【临床意义】

BNP-B型脑钠肽,主要来源于心室。它的含量与心室的压力、呼吸困难的程度、激素调节系统的状况相关。心室的体积和压力增高可导致血浆内BNP升高,升高的程度与心室扩张和压力超负荷成正比。可敏感和特异性地反映左心室功能的变化。用BNP可以协助诊断心力衰竭,判断病情的严重程度和预后。

鉴别诊断呼吸困难:呼吸困难多由心肺疾病引起,鉴别“心源性哮喘”与“肺源性哮喘”经常是令临床医生棘手的问题。BNP能快速鉴别呼吸困难的病因。BNP诊断心力衰竭敏感而且特异,可作为呼吸困难鉴别诊断的一个指标。当BNP<100 pg/ml时,排除心衰;当BNP>400 pg/ml时,考虑心衰;当BNP结果在100~400 pg/ml之间灰区时,考虑慢性阻塞性肺病(COPD)、肺栓塞(PE)和心衰的代偿期。

早期发现心力衰竭病人:传统的检测方法很难早期对心力衰竭病人做出诊断,而BNP用于无症状性心力衰竭病人的筛选具有相当的价值。随着心衰程度的加重,BNP水平逐渐升高。
BNP对心力衰竭病人的预后有价值: BNP水平在评估心力衰竭的预后和危险分层中具有重要的标志物作用。BNP水平越高,其预后越差。
