Acta Biomater:软骨支撑、血管化和表皮化的气管移植再生术

长段气管缺损的修复和功能重建一直是临床上的一大难题。寻找一种理想的气管移植替代物是解决这一问题的唯一途径。本研究提出了一系列新型的仿生活体气管代用品的构建策略。

长段气管缺损的修复和功能重建一直是临床上的一大难题。寻找一种理想的气管移植替代物是解决这一问题的唯一途径。本研究提出了一系列新型的仿生活体气管代用品的构建策略。

 

针对管状软骨支撑问题,采用了基于高密度软骨细胞培养的软骨片技术,避免了材料引发的炎症反应,从而在自体山羊模型中形成了成熟的软骨样组织。对于上皮化的问题,采用自体移植口腔黏膜上皮,实现了构建的气管腔内黏膜覆盖。最后,采用显微外科技术分离双供血的扁平气管筋膜瓣,实现再生软骨和移植上皮同时稳定的预血管化。通过整合上述策略,成功构建了具有管状软骨支撑的血管化和上皮化的气管替代物,并在山羊模型中成功构建了气管替代物。

 

结果表明,重建后的气管具有与原生气管相似的多层肌-软骨-筋膜-粘膜结构,可以实现稳定的存活和长期的气道功能维持,为长段气管缺损的修复和永久性的功能重建提供了一个有前景的气管替代物。
